Öğe A rare cause of respiratory distress in newborns: Case report of a premature infant with congenital cystic adenomatoid malformations type 3(2014) Reşorlu, Mustafa; Topalo?lu, Naci; Uysal, Fatma; Adam, Gürhan; Karata?, Ozan; Kaymaz, Nazan; Yildirim, ŞuleCongenital cystic adenomatoid malformations (CCAMs) are hamartomatous lesions of pulmonary airway which are characterized by proliferations of terminal brochioles. They are usually infiltrated to one lobe. These patients are during the postnatal period at a level of 80% and adult patients are rare. We report a case of Stocker type 3 CCAM which caused respiratory distress in a premature born infant weighing 1940 g who was delivered by emergency caesarean section. Radiological imaging results revealed CCAM. He subsequently underwent right lobectomy. Pathological examination showed immature airspaces and interstitium, including bronchioles and cartilage as type 3 CCAM. Nörolojik muayenesinde sağ gözde her yöne hareketlerinin kısıtlılık ve pitoz saptandı. Sağ gözde direkt ve indirekt ışık refleksi pozitifti. Olgunun görme keskinliği tam olarak değerlendirildi. Kraniyal magnetik rezonans anjiografi tetkikinde 3 D TOF slab sekansında ,superior oftalmik vende dilatasyon ve kavernöz sinüste sinyal artışı izlendi. Serebral anjiografide (DSA) sağda kavernöz sinüs düzeyinde bilateral eksternal karotid arterin multipl meningeal dalından beslenen sağ inferior petrosal sinüse ve superior oftalmik vene direne olan indirekt KKF saptandı. Transvenöz yolla kavernöz sinüs ve inferior petrosal ven koil embolizasyon yöntemi ile fistülde total oklüzyon sağlandı . Material and method: A total of 110 patients without signs of fetal distress were included in the study: 31 idiopathic oligohydramnios and 29 idiopathic polyhydramnios pregnancies (study group) and 50 normal pregnancies (controls). Doppler investigation of the umbilical artery (UA), middle cerebral artery (MCA), fetal descendant thoracic aorta (DTA) and fetal renal artery (RA) was performed in all patients. Fetal RA resistive index (RI) and pulsatile index (PI) values were measured. Values pertaining to type of birth, newborn weight and APGAR scores were compared. Results: Average patient age, gravidity and week of pregnancy were 25 +/- 4, 1.6, and 37.4 +/- 1, respectively. There were no statistically significant differences between the groups as far as UA S/D, MCA S/D, DTA S/D, DTA RI, DTA PI, and RA S/D measurements were concerned. However, in the oligohydramnios group RA RI and RA PI values were significantly higher than the other two groups. Birth weight in the polyhydramnios group and cesarean section rate due to fetal distress in the oligohydramnios group were significantly higher. Conclusions: In the oligohydramnios group, without affecting fetal distress parameters, Doppler USG evaluation identified an increase in the RA resistance. Also in that group, cesarean rate due to fetal distress during labor was significantly higher than in the remaining two groups. We aimed to investigate the possible role of the compression of vestibulocochlear nerve by AICA loop on vestibulocochlear symptoms and to evaluate by two mostly used MRI-based classifications.\rMethods: In this retrospective study, a total of 358 consecutive patients with either tinnitus or hearing loss in any ear and 22 healthy controls without symptoms were enrolled. The anatomical relationship between AICA and vestibulocochlear nerve in bilateral cerebellopontine cisterns was evaluated in axial 3D-FIESTA sequence.\rResults: According to both MRI classifications, the group without vascular loops was the most common MRI feature in patients with tinnitus and hearing loss. When ears with and without tinnitus and hearing loss were compared in terms of vascular loop findings on MRI, no significant difference was found.\rConclusion: Relationship between vestibulocochlear nerve and AICA loop were not found to be associated with vestibulocochlear symptoms when evaluated by two MRI-based classifications. Treatment decisions for vascular compression syndrome should not be based on radiologic findings alone. Possible effects of AICA on vestibulocochlear nerve should be investigated in further prospective randomized studies.Öğe Association of Congenital Left Renal Vein Anomalies and Unexplained Hematuria: Multidetector Computed Tomography Findings(Karger, 2015) Resorlu, Mustafa; Sariyildirim, Abdullah; Resorlu, Berkan; Sancak, Eyup Burak; Uysal, Fatma; Adam, Gurhan; Akbas, AlpaslanObjectives: To investigate whether congenital renal vein anomalies are involved in the etiology of hematuria by analyzing abdominal multidetector computed tomography (MDCT) results. Methods: Six hundred and eighty patients undergoing MDCT for various abdominal pathologies in whom possible causes of hematuria were excluded were retrospectively assessed in terms of left renal vein anomalies, such as circumaortic left renal vein (CLRV), retroaortic left renal vein (RLRV) and multiple renal vein (MRV). Patients with CLRV, RLRV or MRV and patients with normal left renal veins were compared in terms of the presence of hematuria. Results: Left renal vein anomalies were detected in 100 patients (14.7%). RLRV, CLRV and MRV were identified in 5.4, 2.5 and 6.8% of patients, respectively. Hematuria was determined in 8.1% of patients with an RLRV anomaly and in 10.5% of patients with no RLRV anomaly (p = 0.633). Hematuria was detected in 23.5% of patients with a CLRV anomaly and 10.1% of those without (p = 0.074), and in 21.7% of patients with an MRV anomaly and 9.6% of those without (p = 0.009). Condusions: In addition to increasing risk of complication during retroperitoneal surgery, numeric congenital renal vein anomalies are also significant in terms of leading to clinical symptoms such as hennaturia. Karger AG, BaselÖğe Borderline ovarian tumors: a study of 100 cases from a Tertiary Care Hospital(Termedia Publishing House Ltd, 2013) Uysal, Ahmet; Mun, Semih; Uysal, Fatma; Oztekin, Murat; Buyuktosun, Cem; Sehirali, Salim; Basogul, OmerAim of the study: The purpose of the study was to evaluate patients with borderline ovarian tumors. Material and methods: Clinical features, treatment and survival status of 100 patients with borderline ovarian tumors were retrospectively evaluated between 1998 and 2007. Results: Patients mean age was 37.75 years (range: 15-72); 22 of them were postmenopausal. Histopathological diagnoses were serous, mucinous, endometrioid and clear cell in 54%, 41%, 2% and 3% of the patients, respectively; 70 patients had stage IA disease, 8 were at stage IB, 16 at stage IC, 2 at stage IIIA, 3 at stage IIIB and 1 at stage IIIC. Restaging laparotomies were performed on 19 patients; fertility-sparing surgery was performed on 52 patients; 2 patients received chemotherapy because of advanced-stage disease. All patients are currently alive. The 5-year disease-free survival rate for 71 cases was 100%. Conclusions: Borderline ovarian tumors have excellent prognoses, and fertility-conserving surgery can be performed in young patients with early-stage disease.Öğe Cesarean Scar Pregnancy Diagnosis, Management, and Follow-up(Wiley, 2013) Uysal, Fatma; Uysal, Ahmet; Adam, GurhanCesarean scar pregnancy is a very rare form of pregnancy and a life-threatening situation. However, there are haemorrhages on her left psoas muscle. We evaluated these findings as haemorrhagic myositis, which is the rare situation seen during follow up such these patients.Öğe Efficacy and safety of breast radiothermometry in the differential diagnosis of breast lesions(Termedia Publishing House Ltd, 2014) Caferova, Sahnaz; Uysal, Fatma; Balci, Pinar; Saydam, Serdar; Canda, TulayAim of the study: The aim of this study is to research the contribution of radiothermometry (RTM) to the characterization of breast masses, the differentiation of malignant-benign masses and diagnosis of early stage breast cancer. Material and methods: This prospective study comprised 182 cases of patients diagnosed with a breast mass and a control group of 55 cases: a total of 237. Results: When histopathology is accepted as the gold standard among diagnostic methods, the sensitivity, specificity, and positive-negative predictive value for RTM were 90.9%, 20.8%, 61.2% and 62.5%, respectively. Consistency was 0.129. When compared with mammography the same values for RTM examination are 87%, 81.4%, 58% and 95.5%. Consistency was 0.582. Evaluating with respect to size of the mass accepting mammography as the gold standard, RTM examination had sensitivity, specificity, positive and negative predictive values for masses 2 cm and above of 88.9%, 83.3%, 88.9% and 83.3%. Consistency was 0.722. The consistency of RTM for lesion diagnosis in BI-RADS II breast structure is higher than the consistency of mammography. Conclusions: Identification of lesions in the breast and presence of microcalcification by RTM shows that it is more trustworthy compared to mammography. When compared with mammography the validity results for RTM show there is a good level of conformity between the two methods. When evaluated based on the area below the ROC cure and compared to mammography, RTM is sufficiently successful at evaluating positive and negative cases.Öğe Emergency cervical cerclage: effect on pregnancy outcome and mode of delivery(Taylor & Francis Ltd, 2014) Cavus, Yunus; Uysal, Ahmet; Balsak, Deniz; Acar, Zuat; Ince, Zehra; Uysal, FatmaAim: To evaluate the effectiveness of emergency cervical cerclage (ECC) and its effect on the mode of delivery. Patients and methods: Between April 2007 and July 2011 patients attending MH Diyarbakir Obstetrics and Gynecology Hospital and MH Seferihisar State Necat Hepkon Hospital who underwent ECC were included in this retrospective analysis. Results: At the time of ECC, the mean gestational age, cervical length and cervical dilatation were 21.4 +/- 2.2 weeks, 4.3 +/- 0.8 cm and 11 +/- 2.4 mm, respectively. The average time between the procedure and birth was 13.8 +/- 4.9 weeks and this period was sufficient to maintain a viable pregnancy (p < 0.05). In this study, 12 patients (60%) were delivered vaginally and eight (40%) patients delivered with CS (40%). the difference was statistically insignificant (p = 0.371). Regarding the gestational age at time of delivery, 55% of patients delivered at 36 weeks, 70% at 32 weeks, and 80% of them delivered at 28 weeks. The total live birth rate was 90%. Conclusion: ECC provides satisfactory time for the fetus to gain sufficient viability. Pregnancies with emergency cerclage show no difference in terms of birth method, whether cesarean or vaginal birth, and delivery type does not appears to be linked to ECC.Öğe Endovascular treatment of delayed hemorrhage developing after the pancreaticoduodenectomy procedure(Springer Wien, 2014) Adam, Gurhan; Tas, Sukru; Cinar, Celal; Bozkaya, Halil; Adam, Fusun; Uysal, Fatma; Resorlu, MustafaDelayed hemorrhage after pancreaticoduodenectomy (PD) is still one of the most common causes of mortality. However, the case series regarding interventional treatment of delayed hemorrhage after PD are limited. In this retrospective study, we aimed to evaluate functional outcomes of interventional treatment of late hemorrhages developing after PD. We retrospectively evaluated 16 patients who received endovascular treatment for delayed arterial hemorrhage after PD procedure. Postsurgical nonhemorrhagic complications, time of hemorrhage, site of hemorrhage, endovascular treatment technique, postprocedural complications, and mortality rates were obtained. Mean duration of delayed hemorrhage after PD was 18 days. Computed tomography angiography images for the hemorrhage period were available for 15 patients. We observed extravasation alone in seven patients and pseudoaneurysm alone in five. Pushable coil was used in 15 patients and covered stent in 1. Two patients died due to hepatic failure, and one patient died because of multiple organ dysfunction syndrome (MODS). Delayed hemorrhage after PD is difficult to identify, but accurate and early diagnosis is of vital importance. To date, most appropriate management of this complication remains unclear. Although endovascular treatment techniques may vary for every patient, it is a reliable and effective method for halting hemorrhage. Therefore, interventional procedures must be primarily considered rather than surgical interventions.Öğe Epicardial Adipose Tissue Is Increased in Patients With Inflammatory Bowel Disease(Amer Inst Ultrasound Medicine, 2016) Uysal, Fatma; Akbal, Erdem; Akbal, Ayla; Cevizci, Sibel; Arik, Kasim; Gazi, EmineObjectives-Patients with inflammatory bowel disease (IBD) have high cardiovascular morbidity, and, in general, epicardial adipose tissue thickness is related to atherosclerotic vascular disease. This study aimed to investigate the association between epicardial adipose tissue thickness and carotid intima-media thickness as markers of early atherosclerosis in patients with IBD. Methods-The study comprised 47 patients with IBD (25 with Crohn disease and 22 with ulcerative colitis) and 35 control participants. In all individuals, epicardial adipose tissue and carotid intima-media thickness values were measured by sonography. Results-The mean age +/- SD of the 47 patients with IBD was 42.3 +/- 11.2 years, versus 41.4 +/- 10.1 years for the control group. The epicardial adipose tissue thickness was higher in both the Crohn disease and ulcerative colitis groups compared to the control group (P < .001), but not the carotid intima-media thickness (P = .695 and .917, respectively). There was a strong positive correlation between the carotid intima-media and epicardial adipose tissue thickness values in the Crohn disease and ulcerative colitis groups (r = 0.757; 95% confidence interval, 0.711-0.901; r = 0.786; 95% confidence interval, 0.364-0.615; both P < .001). However, there was no significant difference between the patients who were in the active and inactive disease periods in both groups in terms of carotid intima-media and epicardial adipose tissue thickness values. Conclusions-Our findings suggest that epicardial adipose tissue thickness might be a marker for detection of early atherosclerosis in patients with IBD. There was a strong positive correlation between carotid intima-media thickness and epicardial adipose tissue thickness values in the patients with IBD. However, there was no correlation between IBD activity and carotid intima-media or epicardial adipose tissue thickness.Öğe Epicardial adipose tissue thickness in patients with ankylosing spondylitis(Springer London Ltd, 2015) Resorlu, Hatice; Akbal, Ayla; Resorlu, Mustafa; Gokmen, Ferhat; Ates, Can; Uysal, Fatma; Adam, GurhanThe purpose of our study was to measure epicardial adipose tissue (EAT) thickness as a novel indicator of atherosclerosis and cardiovascular risk factor in ankylosing spondylitis (AS) patients and to show the relationship with clinical parameters and inflammatory markers. Forty AS patients (42.75 +/- 12.43 years) and 40 healthy individuals with no cardiovascular risk factor as the control group (43.02 +/- 14.78 years) were included in the study. Carotid intima-media thickness (CIMT) and EAT thickness were measured in AS patients and the control group. Total cholesterol, high-density lipoprotein (HDL) cholesterol, low-density lipoprotein (LDL) cholesterol, glucose, erythrocyte sedimentation rate, urea, and blood pressure were investigated in both groups. In addition, the Bath Ankylosing Spondylitis Functional Index (BASFI) and the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) were used to evaluate the association between clinical findings and CIMT and EAT in the patient group. CIMT and EAT thickness were higher in the AS patients compared to the control group. CIMT was 0.76 +/- 0.19 and 0.57 +/- 0.12 mm (p < 0.001) and EAT thickness was 4.35 +/- 1.56 and 3.03 +/- 0.94 mm (p < 0.001) in the AS and control groups, respectively. A correlation was determined between EAT thickness and CIMT. Triglyceride level, patient age, blood pressure, and duration of disease were correlated with both CIMT and EAT thickness. Diagnosis of ergotamine intoxication begins with suspicion. Recently, the relationship between atherosclerosis and adrenal incidentalomas has been noted. Epicardial fat tissue (EFT) and carotid artery intima-media thickness (CIMT) measurements show the development of atherosclerosis (AS). The aim of this study is to evaluate the risk of cardiovascular disease in AI patients using EFT and CIMT readings. Materials and methods: Our study included 63 AI patients and 48 healthy controls. All patients were evaluated for blood pressure, Body Mass Index(BMI), EFT, CIMT, and lipid parameters. AI patients were also investigated for hormonal secretion. The relationship between risk of atherosclerosis and EFT, CIMT was analyzed. Results: Control subjects (mean EFT, 3.92 ± 1.14 mm) had lower EFT than patients with subclinical Cushing's syndrome (SCS) and non-functional adenoma (NFA) (mean EFT, 6.22 ± 1.97 mm and 5.44 ± 1.61 mm, respectively) (both P < 0.001). There was no difference in EFT between SCS and NFA patients. CIMT was significantly lower in control subjects (mean CIMT, 0.65 ± 0.11 mm) than in NFA patients (mean CIMT, 0.75 ± 0.18 mm) (P < 0.01). The EFT of SCS patients was not significantly different from the EFT of either NFA patients or controls. Hypothyroidism can develop in patients with Graves' disease, either spontaneously or as a result of radioactive iodine therapy or surgery. However, it is rare for patients with Hashimoto's thyroiditis to subsequently develop Graves' disease. We report a case of alopecia areata associated with Graves' disease in a 41-year-old woman who had previously been diagnosed with Hashimoto's disease.
