Although ganglioneuromas are benign, the treatment is surgical as they can cause pain or compression symptoms, can be locally aggressive and can lead to cord compression. We present a young adult female with a ganglioneuroma of the right posterior mediastinum who presented with lower back pain, together with the clinical features, Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) findings, differential diagnosis tips and their contribution to surgical planning.Öğe An uncommon cause of multiple pulmonary nodules; hereditary hemorrhagic telangiectasia(Turkish Assoc Tuberculosis & Thorax, 2016) Kosar, Sule; Kizildag, Betul; Canan, Arzu; Karatag, Ozan; Gonlugur, Ugur; Sariyildirim, AbdullahHereditary hemorrhagic telangiectasia ( HHT), or Rendu-Osler-Weber syndrome ( ROWS) is a very rare hereditary disease. The diagnosis is based on the clinical findings such as recurrent epistaxis, telangiectases, visceral arteriovenous malformations (AVMs) and family history. AVMs are found in the liver, lung or brain and could mimick the masses of these organs. Radiologic evaluation plays a critical role during diagnostic and therapeutic management of ROWS. Hence, radiologists should be aware of the diagnosis of HHT in the patients with AVMs, history of epistaxis and family history. We report a patient with multiple pulmonary AVMs secondary to HHT who has referred to our interventional radiology department for computed tomography guided transthorasic lung biopsy procedure with suspicious of malignancy.Öğe Atypical spinal tuberculosis: posterior paravertebral spread(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2016) Komurcu, Erkam; Kizildag, Betul; Kosar, Sule; Akman, TarikSpinal tuberculosis phenomenon that is an extra pulmonary form of tuberculosis infection, the prognosis of which changes dramatically by early diagnosis and treatment, is still a significant health problem. While early diagnosis and treatment of typical spinal tuberculosis progressing typically is performed without any problems, incorrect treatment of spinal tuberculosis cases presenting atypical findings can lead to dramatic results despite the improvement in modern imaging techniques. In our study we aimed to present an atypical spinal tuberculosis case, for which a correct diagnosis based on abscess and fistula on the thigh that is unique in literature with accompanying pseudomonas co-infection presenting as an undefined atypical spread.Öğe CT and MRI findings of renal infestation by a huge active hydatid cyst(2013) Kizildag, Betul; Dagistan, Emine; Gurel, Safiye; Alan, CabirHydatid cysts derived from a type of tapeworm called Echinococcus granulosis larvaes which can situate in various organs or tissues in human body. It encounters as an endemic zoonosis in many regions all over the world including eastern part of Turkey. Renal involvement of hydatid cysts is uncommon even in endemic areas. The imaging properties vary according to the phase of the disease. Although it is a benign condition, the diagnosis of a renal hydatid cyst is critical in managing treatment and complications, such as nephrectomy, medical treatment before surgery and the risk of anaphylaxis or dissemination during intervention. Herein authors reported a case of an isolated involvement of the right kidney by a huge active hydatid cyst in a young man who was treated surgically, emphasising its ultrasound, CT, contrast-enhanced MR and diffusion-weighted imaging findings.Öğe Dural sinus filling defect: Intrasigmoid encephalocele(2013) Karatag, Ozan; Cosar, Murat; Kizildag, Betul; Sen, Halil MuratFilling defects of dural venous sinuses are considered to be a challenging problem especially in case of symptomatic patients. Many lesions have to be ruled out such as sinus thrombosis, arachnoid granulations and tumours. Encephalocele into dural sinus is also a rare cause of these filling defects of dural sinuses. Here, we report an extremely rare case with spontaneous occult invagination of temporal brain tissue into the left sigmoid sinus and accompanying cerebellar ectopia. Gaucher's disease (GD) is a lysosomal storage disorder that has progressive course and is rarely seen. Research has shown that a pathological femoral neck fracture with GD mostly emerges in childhood. But in adults, there are no reports of pathological femoral neck fractures with GD. We present a unique case of GD with a pathological femoral neck fracture in a 54-year-old woman who did not undergo surgery because of haematological problems including thrombocytopaenia. Furthermore symptoms of musculoskeletal involvement of TB infection are non-spesific, relatively slow and indolent. The diagnosis of TB is less considered by clinicians among other common causes of shoulder pain including traumatic and degenerative conditions. Association of IAD with Hashimoto's disease (HD) is rare. This suggests the possibility of common autoimmune process affecting both the pituitary and the thyroid glands. Tendon xanthomas are diagnostic for heterozygous familial hypercholesterolaemia (HFH) and they mostly occur in Achilles tendon. Sonography and MRI are superior to clinical assessment and are useful in detecting tendon xanthomas. Visual field defects are seen in nearly 90% of the cases. We report a 32-year-old woman who was referred to our clinic with the complaint of longstanding blurred vision and nasal visual field defect in her both eyes. The diagnosis of ODD was made on the basis of clinical appearance of the optic nerves and confirmed by ancillary tests. Binasal visual field defect was observed in the automated visual field test. In conclusion, ophthalmologists should consider ODD in the differential diagnosis of advanced visual field loss and binasal hemianopia.Öğe Severe digital necrosis in a 4-year-old boy: Primary Raynaud's or jellyfish sting(2013) Binnetoglu, Fatih Koksal; Kizildag, Betul; Topaloglu, Naci; Kasapcopur, OzgurRaynaud's phenomena is a common disorder which may be primary or secondary to some connective tissue disorders such as systemic sclerosis and systemic lupus erythematosus. Jellyfish sting is a rare but lifethreatening cause of Raynaud's phenomena. Digital gangrene is reported in 3% of children with secondary Raynaud's phenomena but does not occur in children with primary Raynaud's phenomena. We report a case of a 4-year-old boy who initially presented with episodes of pain and bluish to blackish discolouration and necrosis affecting the fingers on both hands after a jellyfish sting without any sign of connective tissue disorder. All rights reserved.Öğe The Prevalence of Incidental Adrenal Mass Found Using Diagnostic Imaging Techniques(Galenos Yayincilik, 2013) Sen, Hacer; Asik, Mehmet; Uysal, Fatma; Kizildag, Betul; Binnetoglu, Emine; Gunes, Fahri; Erbag, GokhanPurpose: Incidentally-found adrenal masses detected by imaging studies performed for unrelated reasons have become more common in the clinical practice. Our study aims to assess the nature and prevalence of incidental adrenal masses discovered on CT or MRI in patients without malignancy. Material and Method: We analyzed the reports of 8378 abdomen or chest CT and 820 abdominal MRI examinations performed on 4973 patients in our hospital. We found 629 (12.6%) incidental adrenal masses. We excluded 194 (3.9%) patients with known cancer or high-risk adrenal metastasis. The remaining 435 (8.7%) adrenal masses constituted the study group. This group consisted of 274 (5.9%) patients [123 (44.9%) women and 151 (55.1%) men] with masses greater than 1 cm in diameter and 161 (2.8%) patients with masses less than 1 cm. Results: The mean age of the patients was 62.55 +/- 13.23 years. Unilateral adenoma was the most common type of adrenal masses (n= 112, 40.9%). Less commonly observed adrenal masses were: unilateral [n= 79 (28.8%)] and bilateral [n= 35 (12.8%)] macronodular hyperplasia, unilateral [n= 33 (12%)] and bilateral [n= 2 (0.7%)] diffuse adrenal thickness, bilateral adenoma [ n= 7 (2.6)], unilateral [n= 3 (1.1%)] and bilateral [ n= 3 ( 1.1%)] micronodular hyperplasia. The masses were most commonly found in the left adrenal gland (165, 60.2%). They were found at lower rates in the right adrenal gland (66, 24.1%) and bilateral adrenal glands (43, 15.7%). The mean sizes of left and right adrenal masses were 1.89 +/- 1.11 cm and 2.02 +/- 0.86 cm, respectively. Discussion: Patients harbouring adrenal incidentalomas should be evaluated for the possibility of malignancy and/ or hormone activity. The lack of controlled studies impedes specific management and recommendations for adrenal incidentalomas. Large prospective controlled studies on this topic are needed.Öğe Wernicke's Encephalopathy Following Gastrointestinal Surgery: MRI and DWI Findings(Turkish Neurological Soc, 2014) Kizildag, Betul; Canan, Arzu; Sen, Halil Murat; Tas, Sukru; Cikman, Oztekin; Dagistan, EmineWernicke's encephalopathy (WE) is a neurologic disorder resulting from thiamine (vitamin B1) deficiency characterised by the ocular signs, altered consciousness and ataxia. It is usually associated with chronic alcohol abuse. Recently, it emerged as a complication following bariatric surgery. The classical clinical triad occurs in a small group of patients, and early replacement of thiamine is an important prognostic factor. Hence, magnetic resonance imaging (MRI) is useful to support the diagnosis. In this case, we report MRI and diffusion weighted imaging findings of WE in a patient without history of alcoholism after gastrointestinal surgery.
