    The Effect of Kangaroo Care on Breastfeeding and Development in Preterm Neonates

    Purpose: This study investigated the effect of kangaroo care (KC) on breastfeeding rate and development in preterm neonates in the first six months of life. Design: The study was conducted using a quasi-experimental method with a pretest-posttest control group design. The sample consisted of preterm neonates in the NICU of two state hospitals of Turkey. The experimental group (n =30) was provided with KC by their mothers for 30 min once a day during a postnatal month. The control group (n = 30) received routine care. Feeding patterns and physical development parameters were determined during the transition to exclusive breastfeeding and at discharge, and in the first, third, and sixth postnatal months. Data were collected using a descriptive characteristics questionnaire, a nutrition and physical development follow-up form, and a home KC follow-up form. Percentage distribution, mean, chi-square test, and t-test were used for analysis. Results: The KC group had a significantly higher mean body weight than the control group in the third and sixth postnatal months (p < .05). The KC group had higher breast milk intake and breastfeeding rates than the control group during the transition to exclusive breastfeeding and at discharge, and in the first, third, and sixth postnatal months (p < .05). Conclusion: The KC neonates were breastfed for a more extended period of time than controls. This result suggests that KC improves breast milk intake and breastfeeding rates. Therefore, mothers should be trained in KC in the postnatal period. Practice implications: Healthcare professionals should encourage and follow mothers for kangaroo care.

    HEMŞİRELİK ÖĞRENCİLERİNİN KLİNİK KARAR VERME SÜRECİNDE ÖZGÜVEN VE ANKSİYETE DÜZEYLERİNİN BELİRLENMESİ: İKİ FARKLI PROGRAM ÖRNEĞİ

    Amaç: Bu çalışma, intörnlük programı olan ve olmayan iki hemşirelik eğitim programındaki öğrencilerin klinik uygulamalardakarar verme sürecinde özgüven ve anksiyete düzeylerinin belirlenmesi amacıyla yapıldı.Yöntem: Tanımlayıcı ve karşılaştırmalı olarak yapılan bu çalışmanın evrenini, 2017-2018 eğitim öğretim yılı bahar dönemindeöğrenim gören, intörn hemşire uygulaması olan (B okulu) ve olmayan (A okulu) iki farklı üniversitenin 3. ve 4. sınıf hemşireliköğrencileri oluşturdu. Örneklem seçimine gidilmeden araştırmaya katılmayı kabul eden, klinik uygulamanın ilk ve son günüsoru formlarını eksiksiz dolduran öğrenciler ile çalışma tamamlandı. Veriler “Sosyodemografik Veri Toplama Formu” ve“Hemşirelikte Klinik Karar Verme Özgüven ve Anksiyete Ölçeği” ile toplandı.Bulgular: Çalışma sonucunda, intörnlük programında eğitim gören öğrencilerin özgüven toplam ve alt ölçek puanortalamalarının intörnlük programında eğitim görmeyen öğrencilerin toplam puan ortalamalarından istatistiksel olarak dahayüksek bulundu (p<0.05). İntörn programı olan öğrencilerin anksiyete toplam ve alt ölçek toplam puanlarının ise intörnprogramı olmayan öğrencilerin toplam puanlarından daha düşük olduğu, ancak aradaki farkın istatistiksel olarak anlamlıbulunmadığı saptandı (p>0.05).Sonuç: Çalışmada, intörnlük programı hemşirelik öğrencilerinin klinik karar vermede özgüven düzeyini yükseltmede etkili birprogram olduğu sonucu bulundu. Çalışma, ülkemizde konu ile ilgili ilk çalışma olup, yeni çalışmalara ihtiyaç vardır.

    Paternal and Maternal Holding-Cuddling for Procedural Pain in Healthy Term Neonates

    Objective: Holding-cuddling helps reduce procedural pain. Studies have focused on the effect of the maternal holding-cuddling (MHC) method on heel prick pain in healthy-term neonates. However, there is limited study on the impact of paternal holding-cuddling (PHC) on procedural pain during heel pricks in neonates. This paper looked into the impact of MHC and PHC on heel prick pain and crying duration in healthy-term neonates. Methods: This quasi-experimental study occurred in the obstetrics clinic of a university hospital in western T & uuml;rkiye. The sample comprised 92 full-term neonates and their parents, divided into three groups: control (n=32), MHC (n=30), and PHC (n=30). The data were collected using a personal information form, a Neonatal Follow-up Form, and the Neonatal Infant Pain Scale (NIPS). The study received approval from an ethics committee. Results: The MHC and PHC groups had a lower NIPS score than the control group during and after the procedure (P<.05). The MHC and PHC groups had a lower crying duration than the control group (P<.05). There was no difference in NIPS scores and crying duration between the MHC and PHC groups (P>.05). Conclusion: Full-term infants held and cuddled by their parents tend to feel less pain and cry less during heel pricks. Nurses should use MHC and PHC to help infants have less pain during heel pricks.

    The effect of kangaroo care on cortisol levels and immune factors in breast milk

    This paper investigated the effect of kangaroo mother care (KMC) in the early postpartum period on cortisol levels and immune factors in breast milk. This quasi-experimental study was conducted at the obstetrics clinic of a university hospital in western Türkiye. The sample consisted of 63 mothers and their infants. All mothers had a cesarean delivery. Participants were divided into control (n = 32) and experimental groups (n = 31). The control group received routine care at the clinic. The experimental group received KMC for the first 3 days after birth in addition to the routine care at the clinic. Milk samples were collected on the third day after delivery to examine cortisol, IgA, IgM, and IgG levels. All parameters were measured using the enzyme-linked immunosorbent assay method. The experimental group had lower cortisol levels (17.740 ± 1.438) than the control group (18.503 ± 1.449) (p < .05). This result showed that the difference between the two groups was clinically significant (effect size = .53). There was no significant difference in IgA, IgM, and IgG levels between the groups (p > .05). The experimental and control groups had similar immunological factors, but the former had lower cortisol levels than the latter. Therefore, healthcare professionals should encourage mothers to provide KMC to their infants as soon as possible.
