    The Effect of the Ovarian Endometrioma Surgery on Ovarian Reserve and Ovarian Blood Flow

    Aim:To investigate the effectiveness of laparoscopic endometriosis surgery on ovarian reserve and ovarian blood flow index. Material and Method: Twenty-six patients were evaluated prospectively whom had laparoscopic surgery due to endometrioma in third care education and research hospital. Before and after surgical procedure all patients FSH, LH and E2 levels were measured in their 3rd menstrual period day. Periodically cysts and ovarian volume as well as ovarian artery resistive index measured by using transvaginal Doppler sonography. Results:Twenty-six patients' mean age was 25, 6 +/- 4, 5 (20-36 years). When the compared to changes of preoperative and postoperative values of hormonal parameters FSH (p:0,455) and LH (p:0,672) were insignificant and E2 (p:0,024) was significant respectively. While ovarian volume was decreasing significantly (p:0,001), Ovarian Artery RI parameter was not changed (p:0,587). Discussion:Our study was showed that while laparoscopic surgical treatment of ovarian endometriomas significantly decrease ovarian volume, but hormonal parameters and ovarian blood flow were not affected.

    The Effects of Hysteroscopic Submucosal Myoma Resection on Number of Sanitary Pads

    Aim: To research the effect of hysteroscopic resection of submucosal myoma (HRSM) on the amount of pads used during the menstrual period in a premenopausal patient with menometrorrhagia caused by submucosal fibroids. Material and Method: A total of 27 women with submucosal fibroids were included in this prospective study using the Pictorial Blood Assessment Chart (PBAC) to evaluate quantity of menorrhagia. Bleeding evaluation and number of pads used by patients in the 6th month after HRSM were recorded. Results: The average age of patients included in the study was 41 +/- 5.1. Of 27 patients 20 (74%) had one. 4 (14%) had two and 3 (12%) had three or more submucosal myomas. The diameter of removed fibroids were 17 (63%) less than 3 cm, 6 (22%) between 3 and 5 cm and 4 (15%) were above 5 cm. In the postoperative 6th month on the day of heaviest menstrual bleeding average number of pads used dropped to 3.8 compared to 9.1 in the preoperative period (p<0.05). Discussion: HRSM performed due to menometrorrhagia in the premenopausal period significantly reduces the number of pads used and the number of days of bleeding.

    The expression and prognostic significance of the tumour-associated glycoprotein 72 in the normal, hyperplastic and neoplastic endometrium using immunohistochemical methods

    Aim of the study: Our aim was to investigate the expression and the prognostic significance of tumour-associated glycoprotein 72 (TAG-72), which is an epithelial tumour marker, using immunohistochemical methods in the normal, hyperplastic and neoplastic endometrium. Material and methods: The study group comprised of 47 cases with simple non-atypical hyperplasia and normal endometrium (n = 17), complex non-atypical hyperplasia (n = 10), and endometrial adenocarcinoma (n = 21). The epithelial tumour marker TAG-72 was determined by immunohistochemical methods. The location of TAG-72 in the differential diagnosis and its prognostic value were statistically analyzed. Results: The immunoreactivity of the immunohistochemical dye was evaluated in terms of the pattern, intensity and pattern + intensity. The comparison between the three groups showed that there was a significant difference in terms of pattern (p < 0.001), intensity (p < 0.001) and pattern + intensity (p < 0.001). The marker TAG-72 was not found to be a prognostic marker for endometrial adenocarcinoma. Conclusions: The marker TAG-72 is a reliable parameter and marker for the differentiation of the neoplastic endometrium from the non-neoplastic and hyperplastic endometrium. There is a significant relationship between the increasing age and staining pattern of the TAG-72 expression. Still, TAG-72 does not seem to be an independent prognostic factor for the endometrial carcinoma.

    The impact of thrombocytosis on clinicopathological prognostic factors and survival in patients with vulvar cancer

    Purpose: Reactive thrombocytosis in many solid tumors has widely been studied. In the present study we aimed to investigate whether thrombocytosis is a common and prognostic factor in women with vulvar cancer. Material & Methods: The preoperative platelet counts of 41 women, treated for vulvar cancer in our onco-gynecology center between March 1994 and January 2007, were retrospectively reviewed and correlated to clinical and pathological prognostic factors and 5-year survival. The chi-square or Fisher exact tests were used to compare categorical variables. P value <0.05 was accepted for statistical significance. Results: The mean age was 65.4+/-11.3 years (range 39-83y). All patients had squamous histology. The mean platelet count was 335.42x109/L +/- 82.03 (range 142-1155x109/L). Thrombocytosis was detected in 8 (19.5%) patients. No correlation was found between thrombocytosis and grade (p=0.65), LVSI (p=0.82), tumor size (p=0.73), depth of invasion (p=0.18), lymph node metastasis (0.93), and FIGO stage (p=0.78). The mean follow up time was 118.0+/-43.1 months (range 60-213 months). At the end of the study period 14 patients (34.2%) had died, 8 (19.5%) had recurrence, 19 (46.3%) were disease-free. General 5-year survival was 68.3% (28/41). The 5-year survival rate for patients with thrombocytosis was 75.0% (6/8), which was not significantly different from the 5-year survival of patients with normal platelet counts (22/33; 66.7%) (p=0.75). Conclusion: Our study showed that, overall, thrombocytosis was found in about 20% of patients with vulvar cancer and proved to be not linked to the best known prognostic factors and survival. Thus, disease stage and inguinofemoral lymph node status continue to be the best prognostic factors for this disease.

    The Role of Endocrine and Endometrial Factors in Cases of Recurrent Miscarriage: A Tertiary Center Experience

    Aim: To investigate endocrinologic and endometrial factors in cases of recurrent abortions. Material and Method: In cases of recurrent abortions, clinical and ultrasonographic features, genetic, anatomic and immunologic factors, hormonal profiles and endometrial samplings were assessed. Chromosomal abnormalities and uterine anomalies were excluded. Results: In 8 (14%) of 57 cases with recurrent abortions, there were low progesterone levels. In 1 (1.75%) case there was a high androgen level. In 2 (3.5%) cases there was hyperprolactinemia and in another 2 (3.5%) cases there were high insulin levels. In 4 (7%) cases two scores of OGTT were high. In 51 cases where endometrial sampling was performed, only one (1.75%) case had delayed endometrial development. Discussion: We conclude that recurrent abortions have a complex etiology related to endocrinologic and endometrial factors.
