    Comparison of glaucoma diagnosis rates in the tertiary and district clinics

    Between dates 14.11.2012 - 03.09.2013 the patients over the age of 18 were included to the study who applied to the both centers. In patients whose intraocular pressure was found to be elevated and/or optic nerve was found yo be suspect glaucomatous, the glaucoma diagnosis was confirmed by doing computerized visual field and by measuring the retinal nerve fiber layer thickness. Within a year, in both centers the proportion of patients newly diagnosed with glaucoma were compared. Total number of patients who were examined at district polyclinics was 3032 (1406's men and women 1626's) and 17,173 in the university hospital (8019 males and 9154 females) and the mean age was46.7 and 49.3, respectively. While the average age was found statitically higher in faculty hospital (p <0.05), there was not found significant difference between the groups in terms of gender (p> 0.05). The number of newly diagnosed glaucoma patients was found 2 (0.065%) in district polyclinic and 47 (0.27%) in faculty hospital. The proportion of newly diagnosed patients in the university hospital were statistically higher (p <0.05). Even if in the city center, the location and pre-opening promotion of health institutions are also important. We thought that the studies comparing between previously opened, well-known district polyclinics and university hospitals with longer term follow-up would be useful in this topic. © 2014, Anatol J Clin Investig.     The early term effects of cataract surgery on intraocular pressure levels in patients with pseudoexfoliation

    This study was aimed to evaluate the early period of intraocular pressure changes after the cataract surgery applied by phacoemulsification in patients with and without pseudoexfoliation. Twenty-two patients with pseudoexfoliation as a study group and age - gender matched twenty-two patients without pseudoexfoliation as a control group who underwent to cataract surgery with phacoemulsification was included to the study. After cataract surgery, the first week and the first month,of the intraocular pressure changes were analyzed retrospectively in the study and control groups. Preoperative median intraocular pressure measurements were; 15 (5) mmHg in study group and 14,5 (4,3) mmHg in control group. In study and control groups, median intraocular pressure reductions were, respectively, -3 (16) mmHg and -1,5 (3,5) mmHg at postoperative first week, and -2 (4) mmHg and -2 (3,25) mmHg at postoperative first month These values were found statistically significant (p<0.05). However intraocular pressure reductions were not determined as significant different between these groups (p>0.05). Although it was determined a significant decrease in intraocular pressure levels in the early periods after cataract surgery of patients with pseudoexfoliation, this significance was not too different in patients whithout pseudoexfoliation. Therefore cataracat surgery may provide additional benefit in the management of glaucoma patients especially due to pseudoexfoliation.

    The relationship between tear osmolarity, ocular surface disease index© and other tear function tests

    Objective: To evaluate the relationship between tear osmolarity, Ocular Surface Disease Index (OSDI© Allergan Inc., Irvine, Calif), Schirmer test and tear break-up time (TBUT). Material and Methods: After performing Ocular Surface Disease Index (OSDI©) survey, tear osmolarity testing with TearLab osmolarity system, Schirmer test without anesthesia and TBUT measurement were performed in 40 patients with dry eye syndrome diagnosis and 39 healthy subjects. Student t-test and Spearman's rho correlation analysis are used for statistical evaluation. Results: The mean tear osmolarity was 307±19.2 mOsm/l in group A, while it was 292.9±13.9 mOsm/l in group B. Schirmer test results were in normal ranges in both groups (13.4±7.5 mm in group A, and 17.4±8.5 mm in group B). TBUT was lower in group A, and the difference between two groups was statistically significant. OSDI© scoring was 43.9±20.4 in group A, while it was 8.56± 5.2 in group B and the difference was statistically significant. The sensitivity and spesificity of tear osmolarity for threshold value 305 mOsm/l were found to be 76.9 % and 80%, for TBUT for 10 seconds were 94.7% and 71.8%; for OSDI© score >15 were 79.5% and 70% and for Schirmer's test for 10 mm were 35.9% and 80%. Conclusion: Of all tests, TBUT, tear osmolarity and OSDI© are found to be successful in discriminating dry eye patients from healthy participants. The specificity and the sensitivity of the commonly used Schirmer test are found to be low. © 2013 by Türkiye Klinikleri.
