    A rare case of fatal venous and cerebral air embolism

    A venous air embolism can occur as a result of circumstances that include blunt head or chest trauma, thoracentesis, arterial catheterization, neurosurgery, cardiac surgery, and Caisson disease. The formation of a venous air embolism requires an air source, interaction between the air source and the vessel, and a pressure gradient supporting air migration into the vessel. Air enters through the impaired venous structure and travels to the right side of the heart and the pulmonary arteries, and depending on the amount of air, may occasionally be fatal. This report is the description of the case of a 3-year-old child who developed a fatal venous and cerebral embolism during neurosurgery for the treatment of skull fractures with epidural and subdural bleeding due to blunt head and chest trauma resulting from a television falling on her. The pathophysiology of death and notes regarding the medico-legal autopsy procedure in such cases are discussed. Meticulous autopsy techniques must be used to determine the presence of an air embolism in cases of blunt trauma, especially in patients with blunt trauma to the head who die during neurosurgery, and possible future malpractice claims should be kept in mind.

    Acute and subacute cardiovascular effects of synthetic cannabinoid JWH-018 in rat

    PurposeThis study investigates the cardiovascular effects of the synthetic cannabinoid naphthalene-1-yl-(1-pentylindole-3-yl)methanone (JWH-018) in rats. The research aims to evaluate the pharmacologic, cardiologic, biochemical, and histopathological effects of acute and subacute administration at low and high doses. The primary research question is how JWH-018 impacts heart function, blood pressure, ECG patterns, and cardiac tissue integrity.MethodsWistar albino rats were divided into five groups: control, acute low-dose (ALD, 0.5 mg/kg), acute high-dose (AHD, 5 mg/kg), subacute low-dose (SALD, 0.5 mg/kg for 14 days), and subacute high-dose (SAHD, 5 mg/kg for 14 days). Cardiovascular effects were assessed using echocardiography, hemodynamic and ECG analysis, histopathology, biochemical markers, and LC-MS/MS quantification of JWH-018 and its metabolites in heart tissue.ResultsAcute high-dose JWH-018 caused bradycardia and hypotension, while subacute high-dose increased heart rate but continued to lower blood pressure. JWH-018 induced cardiac arrhythmias, conduction blocks, and ischemic ECG changes, with prolonged QT intervals in subacute high-dose rats. Histopathological findings revealed myocardial infarction-like features, including contraction bands and ischemic damage, particularly in subacute groups. Elevated pro-BNP and triglycerides indicated cardiac stress and metabolic effects. JWH-018 and its metabolites were detected in heart tissue, primarily in high-dose groups.ConclusionsJWH-018 has significant cardiovascular risks, causing heart rate dysregulation, hypotension, arrhythmias, and ischemic damage. These effects depend on dose and duration. The study highlights the potential dangers of synthetic cannabinoids, emphasizing that they should not be considered safe alternatives to natural cannabis.

    An examination of forensic autopsy cases with pulmonary embolism

    The aim of this study was to examine the frequency, origin, histopathology, and risk factors of PE, the vascular characteristics causing obstruction and the direct or indirecteffect on mortality. A retrospective examination was made of autopsies conducted between 2013 and 2016 in the Malatya Forensic Medicine Group Directorate. Casesdetermined with PE were evaluated in respect of age, gender, event origin, risk factors, history of hospitalization, size of the vessels obstructed by the thromboembolismand histopathological examination. Pulmonary embolism is a frequently seen complication during hospitalization and is one of the leading causes of preventable hospital deaths. When advanced diagnostic methods are used, deep vein thrombosis is determined in 79% of PE patients. The annual incidence in Turkey is approximately15/10,000 cases, which is comparable to rates in western populations. Although PE is seen more at advanced ages, attention must be paid to young cases with trauma-related injuries. Histopathological examination is of the greatest importance as the mortality rate associated with large vessel obstruction is high and obstructions in smallvessels alone also contribute to mortality.

    Evaluation of socio-demographic characteristics of people was determined to use synthetic cannabinoids in Malatya and its surrounding cities / districts and LC-MS/MS analysis method

    Aim: Synthetic cannabinoids (SC) were initially developed in 1960s to target the cannabinoid receptors in medical therapy. However, in recent years, the developed SC derivatives by processing the plant-based material have been sold on websites under the name of natural drugs. The city of Malatya is located at the eastern part of Turkey and has an important location for the transfer and marketing of narcotic substances. In our study, presentation of the socio-demographic characteristics of the cases, the SC was determined in whose blood samples in the analyses, living in Malatya and the surrounding cities-districts were aimed. Materials and Methods: In the scope of study, 275 cases whose blood samples was sent to the Chemistry Department in Forensic Medicine Institute at Malatya Group Presidency in 2016 and were detected as SC-positive by the chemical analysis were investigated. Results: Out of 275 cases, 97.8% were male, 2.2% were female. Median age was 24, 66.5% (n=180) of them were primary school, 18% (n=51) were high school graduate and 0.7% (n=2) were university graduate. The educational status of 27 cases could not be obtained. Conclusion: SC derivatives are marketed with slogan of “the natural ones are harmless”, and their use among the young people is rapidly becoming widespread. In our study, it was determined that the SC users are in the young age group and have low educational status. In order to decrease the number of users, awareness meetings regarding the harmful effects of SC must be organized in the educational institutions.

    Retrospective Analysis of Psychiatry Specialization Theses Made Between 1981-2018 in Turkey

    Objective: In this study, we aimed to qualitatively evaluate the views to the Psychiatry specilization theses, variables affecting publishing of these, and the relationship with the academic career of authors of these theses in Turkey. Method: Theses were searched from the website of Higher Education Board Presidency National Dissertations Center using the terms psychiatry, mental health and diseases. Only the theses with full texts were included in the study. The publications associated with these theses were searched using SCI and SCI-E, Google, Google Scholar and PubMed by using the names of authors and their advisors. Results: We were able to find 910 theses. 748 of the 910 were completed in universities. The overall publication rate of the theses was 37.7%. 19.2% were indexed in PubMed, 28.5% in SCI and SCI-E and 31.9% in Google Scholar. Publication of a thesis was significantly associated with the field of research, the title of the advisor and whether the owner of the thesis had later become a faculty member. Residents who were trained at universities were more likely to publish as a first author. Being first author was associated with later academic career. In addition, those who continued their career as an academic continued to publish on a similar subject more than those who did not. Conclusions: In this study, the type of the research, title of the advisor, enrolling an academic career were associated with the publication of the theses. We hope our results would help better publication of the theses in the future.
