Öğe Çanakkale İlinde Aterosklerotik Kardiyovasküler Hastalıkların İçme Suyu Sertlik Seviyesi ile İlişkisi(2021) Oğuz, SonayAmaç: Kardiyovasküler hastalıklar (KVH) ölüm sebepleri içerisindeki liderliğini halen korumaktadır. Risk faktörleri ve hastalık ilişkisi ile ilgili pek çok çalışma yapılmış ve ispatlanan öneriler güncel kılavuzlara girmiştir. Fakat su sertliği ve KVH ilişkisi ile ilgili literatürde karşıt yorumların olduğu ve halen kesin kanaatlere varılamadığı görülmektedir. Çanakkale şehir merkezinde içme suyu ihtiyacı sertlik derecesi farklı iki ayrı kaynaktan karşılanmaktadır. Bu durum su sertlik seviyesi ve KVH ilişkisinin daha iyi anlaşılmasına yardımcı olabilir. Bu amaçla çalışma planlanmıştır.Gereç ve Yöntem: 2015-2020 yılları arasında Çanakkale Onsekiz Mart Üniversitesi Eğitim ve Uygulama Hastanesi’ne başvurmuş ateroskleroz ileilişkili tanısı olan (koroner arter, karotis arter ve periferik arter hastalığı) Çanakkale il merkezine kayıtlı olgular retrospektif olarak tespit edilerekaterosklerotik (AS) grup oluşturuldu. Bu gruba karşılık gelecek şekilde olguların yaş, cinsiyet, arteriyel hipertansiyon (HT), diabetes mellitus (DM), dislipidemi (DL) mevcut olması durumuna göre, “propencity score matching” gözetilerek non-AS grup oluşturuldu. Yerel yönetimlerden elde edilen il merkezi içme suyu analizlerinden pH, sertlik, kalsiyum, magnezyumun son beş yıllık değerleri tespit edilip adres ile ilişkilendirildi. Bu bilgiler kullanılarak farklı sertlikte su tüketen olgular tespit edildi. İstatistiki olarak iki grup arasındaki su sertlik değeri açısından anlamlılık değerlendirildi.Bulgular: Gruplar arasında yaş, cinsiyet, HT, DM, DL’de anlamlı farklılık yoktu (p>0,05), non-AS grubunda çok yüksek sertlikte su tüketiminin anlamlı seviyede yüksek olduğu tespit edildi (p<0,001).Sonuç: Elde edilen sonuçlar içme suyunun sertlik seviyesi ile ateroskleroz arasında ters bir ilişki olabileceğini göstermektedir. Bilimsel değeri yüksek veriler elde etmek için uygun lokasyonlarda saha çalışmaları ile sonuçların desteklenmesi ve çıkan sonuçlara göre halkın içme suyu konusunda bilinçlendirilmesi gerektiğini şiddetle tavsiye ediyoruz.Öğe Effect of using autotransfusion system on cost in open-heart surgery(Bayrakol Medical Publishing, 2021) Oğuz, SonayAim: The high cost of autotransfusion systems can limit its usage. In our study, we aimed to compare the cost of open-heart surgery cases with and without the use of an autotransfusion system in our hospital.Material and Method: Of the isolated coronary artery bypass grafting (CABG) cases performed, 22 cases (Group 1), in which the autotransfusion system was used and 22 cases (Group 2), in which the autotransfusion system was not used, were retrospectively determined. In particular, the use of blood and blood prod-ucts that will affect costs, the amount of drainage for the first 24 hours, mechanical ventilator, intensive care-hospitalization periods, preoperative hematocrit and postoperative hemotocrit values and demographic data were obtained. Costs between groups were calculated and compared taking into consideration the current prices.Results: In Group 1, the use of erythrocyte suspension (median: 2.0 - 3.0; p = 0.003) and intensive care periods (mean: 1.77 - 2.55; p = 0.046) were found to be significantly lower than Group 2. There were no significant differences between the groups in terms of other variables. When calculating, it was determined that in Group 1, the cost for each case was more than 1050 TL.Discussion: The autotransfusion system is expensive and can provide a reduction in total cost with the benefits it provides. However, it is seen that this does not reduce the total cost enough to save its own costs when considering the current expenses. Compared to the short-and long-term benefits they provide, meeting this cost seems to be tolerable with case selection.Öğe Efficiency of using a neurofeedback device in determining ischaemic early electroencephalography indicators in rabbits with acute brain ischaemia(Oxford University Press, 2021) Oğuz, SonayOBJECTIVES: Continuous electroencephalography (EEG) monitoring is a useful method in surgical procedures in which brain circulation is at risk. Providing this function using neurofeedback devices reduced to small dimensions may provide ease of use in the early diagnosis of brain ischaemia. The goal of this study was to demonstrate the efficiency of using a neurofeedback device in determining the early EEG indicators of ischaemia in a rabbit model of acute brain ischaemia. METHODS: Three randomized groups-carotid ischaemia (CI), global ischaemia (GI) and a sham group-each comprising 8 rabbits, were created. In the CI group, the bilateral main carotid artery was clamped; in the GI group, the bilateral subclavian and main carotid arteries were clamped and brain ischaemia was created for 15 min. Brain reperfusion was then achieved for 30 min. In the sham group, the same surgical preparation was performed but no ischaemia occurred. The brain EEG wave activities of all subjects were recorded during the experiment. At the end of the procedure, all brain tissue was removed and apoptotic indexes were determined by histopathological examination. The statistical significance of the histopathological results and the EEG wave activities among the groups was examined. RESULTS: There was a significant difference between the sham, CI and GI average amplitude ratios, delta (1.02, 0.69, 0.16; P < 0.001) and total wave (0.99, 0.78, 0.49; P < 0.001), respectively. There was no significant difference between the sham and CI groups in delta (sham, CI, 1.01, 0.87; P = 0.1), total wave (sham, CI, 1.22, 0.98; P= 0.2) and amplitude standard deviation rates. However, there was a significant difference in the GI group (P < 0.001). There was a significant difference between all groups in apoptotic index (sham, 17.88; CI, 40.75; GI, 55.88; P < 0.001). CONCLUSIONS: Significant EEG wave changes resulting from experimental brain ischaemia were analysed with the use of a neurofeedback device. The results indicated that the change in the delta and the total wave standard deviations may be an additional indicator in the formation of permanent brain damage.Öğe Femoral Artery and Vein Insulated Injury with Piercing Tool(Çanakkale Onsekiz Mart Üniversitesi, 2019-08-15) Oğuz, Sonay; Gül, Müslim; Başçıl, HasanVascular injuries have high mortality and morbidity rates with active bleeding and shock symptoms. Early and rapid intervention is extremely important in such injuries. In this case report, we present the results of a rapid and coordinated operation of a 19- year-old male patient with right femoral artery and vein were injured by a piercing cutting tool in hypovolemic shock. We aimed to emphasize the importance of rapid and coordinated work with the necessary procedures in this type of stab wounds, the time until the operation from the scene of surgery and the value of emergency primary vascular repair in reducing mortality and morbidity.Öğe Histopathological changes in myocardial tissue due to coronary venous hypertension(Termedia Publishing House Ltd, 2023) Akşit, Ercan; Büyük, Başak; Oğuz, SonayIntroduction: In chronic venous insufficiency (CVI), an increase in venous pressure causes the passage of intravascular blood cells and molecules into the surrounding tissues and induces histopathological changes in the lower extremities, leading to increased pigmentation in the legs, ulceration, and tissue loss to various degrees. This study aimed to investigate whether an increase in venous pressure in the coronary veins can lead to the aforementioned histopathological changes. Material and methods: Twenty-four New Zealand rabbits were divided into the following three groups: experimental model of coronary venous hypertension (CVH) (n = 8), sham group (n = 8), and control group (n = 8). After 21 days postoperatively, tissue samples from each group were compared for perivascular inflammation, erythrocyte extravasation, macrophage infiltration, and hemosiderin deposits by histopathological scoring under a light microscope. Matrix metalloproteinase-2 (MMP-2) activation was evaluated using immunohistochemical staining. Results: In the CVH group, hemosiderin accumulation was significantly higher than in the sham and control groups (1.0 (1.0–3.0), 0.0 (0.0–1.0), 0.0 (0.0–0.0); p < 0.001). Immunohistochemically, in the CVH group, MMP-2 levels were significantly higher than in the sham and control groups (2.0 (1.0–3.0), 0.0 (0.0–1.0), 0.0 (0.0–0.0); p < 0.001). Conclusions: This experimental study showed for the first time the histopathological and immunohistochemical changes in myocardial tissue, similar to those observed in CVI, as a result of increased coronary venous pressure due to coronary vein ligation. Persistent left superior vena cava (PLSVC), an important congenital anomaly that increases coronary venous pressure because of increased coronary venous volume, was found to have a prevalence of 0.29 % as in the literature.Öğe Our mid-and long-term results of endovenous radiofrequency ablation in great saphenous vein insufficiency(Bayrakol Medical Publisher, 2024) Oğuz, SonayAim: Venous insufficiency may occur due to problems in the return of blood to the heart and reverse flows. Endovascular procedures have emerged as an alternative to classical open surgery in treatment. In this study, we aimed to examine the mid- and long-term results of our radiofrequency ablation (RF) applications, which is an endovascular method. Material and Method: In our study, we included 193 patients who underwent RF in our clinic between June 2017 and March 2022. All patients underwent ablation with an RF catheter by entering the great saphenous vein (GSV) at the appropriate level under local tumescent anesthesia and under Dopplerultrasound guidance. Postoperative follow-up of the patients and Dopplerultrasound controls at the 1st, 6th, 12th and 24th months were performed, and GSV patency and reflux conditions were recorded. Results: 133 patients were female (68.9%) and 60 patients were male (31.1%). The age range was 22-82 (mean: 52). It was determined that GSV recanalisation and reflux developed in 6 patients (3.1%) at the 1 -month follow-up, in 9 patients (4.6%) at the 6th month, and in 11 patients (5.6%) at the 12th and 24th months. Discussion: RF method is a feasible method with low complications, high patient comfort and success in suitable patients. However, recanalization and recurrence still occur. To achieve higher success rates, the reasons underlying recanalization and recurrence should be examined.Öğe Systemic capillary leak syndrome during cardiac surgery(Turkish Society of Cardiovascular Surgery, 2023) Aşgün, Halil Fatih; Oğuz, SonaySystemic capillary leak syndrome is defined as excessive fluid and protein extravasation caused by microvascular hyperpermeability. It is a very rare condition, and may occur during or after any surgery without preceding symptoms or a suggestive history. It has been reported in pediatric cardiac surgery patients, and is not expected in adults. In this article, we present a 75-year-old female case of severe systemic capillary leak developed during coronary artery bypass grafting combined with dual valve replacement. To the best of our knowledge, this is the first adult case in the literature having an acute attack of systemic capillary leak syndrome during cardiac surgery.
