Öğe A new technique to simplify the minimally invasive parathyroidectomy: Ultrasound-assisted guided wire localization for solitary parathyroid adenomas(Pakistan Medical Assoc, 2016) Ozkul, Faruk; Arik, Muhammed Kasim; Eroglu, Mustafa; Faydaci, Umut; Toman, Huseyin; Tas, Sukru; Ukinc, KubilayObjective: To investigate the benefits of ultrasound-assisted guided wire localization in MIP for selected cases. Methods: In this prospective, nonrandomised study, we included 36 patients with solitary parathyroid adenomas diagnosed preoperatively by 99m Tc sesta MIBI scintigraphy and/or neck ultrasonography. An ultrasound-guided wire was placed in the solitary parathyroid adenoma preoperatively. MIPs were performed under local anaesthesia plus sedation. After the excision, the parathyroidectomy was confirmed with postoperative ultrasonography. Results: There were 36 patients included in our study. The mean age was 54.89 +/- 11.28 years, and 30 patients were females (83.3%). Preoperative PTH and calcium (Ca) levels were 269.5 pg/mL (83.5-5,000 pg/mL) and 12.2 mg/dL (11.1-20 mg/dL), respectively. Postoperative serum PTH and Ca levels were 42.04 +/- 26.65 pg/mL and 8.95 +/- 0.74 mg/dL, respectively. The mean operation time was 21.69 +/- 6.4 minutes and the average hospitalisation time was 18 hours (range: 10-72 hours). Conclusions: Ultrasound-assisted guided wire localization may be useful in selected MIP cases. The MIP advantages include higher success rates and being easy to learn and practise.Öğe Adropin Levels in Polycystic Ovary Syndrome Patients(Derman Medical Publ, 2017) Sen, Hacer; Erbag, Gokhan; Binnetoglu, Emine; Eroglu, Mustafa; Turkon, Hakan; Tekin, Sati Zeynep; Asik, MehmetAim: Polycystic ovary syndrome (PCOS) is one of the most commonly observed endocrinopathies in women of reproductive age. Women with PCOS are said to have increased classic risk factors for cardiovascular disease, hypertension, dyslipidemia, diabetes, and obesity, in addition to non-classic risk factors such as an increase in C-reactive protein (CRP), homocysteine, and tumor necrosis factor-a. Adropin is a protein thought to play a role in maintaining energy homeostasis and insulin response. The aim of our study is to investigate the relationship between levels of adropin and insulin resistance in PCOS patients with insulin resistance and an increased risk of diabetes. Material and Method: Fifty-seven female patients (30 patients with PCOS and 27 healthy control subjects) were enrolled in this study. All patient's body mass index and insulin resistance were calculated. The adropin levels were measured using commercial kits based on a competitive plasma EIA (enzyme immunoassay) method. Results: The adropin levels in the patient group were 10.79 ng/L, while the value was 13.02 ng/L in the control group, and the difference was statistically significant (p= 0.04). There was a significant negative correlation between the adropin levels and the insulin, aspartate aminotransferase (AST), triglyseride (TG), and homeostasis model assessment of insulin resistance (HOMA-IR) levels (p= 0.03, p= 0.03, p= 0.04, and p= 0.02, respectively). Discussion: In our study, the adropin level which is associated with insulin resistance, was found to be decreased in patients with PCOS. We think that it would be valuable to conduct new studies for the evaluation of adropin related clinical conditions leading to insulin resistance in patients with PCOS.Öğe Decrease in TSH levels after lactose restriction in Hashimoto's thyroiditis patients with lactose intolerance(Springer, 2014) Asik, Mehmet; Gunes, Fahri; Binnetoglu, Emine; Eroglu, Mustafa; Bozkurt, Neslihan; Sen, Hacer; Akbal, ErdemWe aimed to evaluate the prevalence of lactose intolerance (LI) in patients with Hashimoto's thyroiditis (HT) and the effects of lactose restriction on thyroid function in these patients. Eighty-three HT patients taking l-thyroxine (LT4) were enrolled, and lactose tolerance tests were performed on all patients. Lactose intolerance was diagnosed in 75.9 % of the patients with HT. Thirty-eight patients with LI were started on a lactose-restricted diet for 8 weeks. Thirty-eight patients with LI (30 euthyroid and 8 with subclinical hypothyroidism), and 12 patients without LI were included in the final analysis. The level of TSH significantly decreased in the euthyroid and subclinical hypothyroid patients with LI [from 2.06 +/- A 1.02 to 1.51 +/- A 1.1 IU/mL and from 5.45 +/- A 0.74 to 2.25 +/- A 1.88 IU/mL, respectively (both P < 0.05)]. However, the level of TSH in patients without LI did not change significantly over the 8 weeks (P > 0.05). Lactose intolerance occurs at a high frequency in HT patients. Lactose restriction leads to decreased levels of TSH, and LI should be considered in hypothyroid patients who require increasing LT4 doses, have irregular TSH levels and are resistant to LT4 treatment.Öğe Does Retinal Neurodegeneration Seen in Diabetic Patients Begin in the Insulin Resistance Stage?(Turkish Ophthalmological Soc, 2016) Arikan, Sedat; Ersan, Ismail; Eroglu, Mustafa; Yilmaz, Mehmet; Tufan, Hasan Ali; Gencer, Baran; Kara, SelcukObjectives: To investigate whether retinal neurodegeneration and impairment in contrast sensitivity (CS), which have been demonstrated to begin in diabetic patients before the presence of signs of diabetic retinal vasculopathy, also occur in the stage of insulin resistance. Materials and Methods: The average, minimum and sectoral (inferior, superior, inferonasal, superonasal, inferotemporal and superotemporal) thicknesses of the ganglion cell-inner plexiform layer (GCIPL) measured using optical coherence tomography were compared between an insulin-resistant group and control group in order to evaluate the presence of retinal neurodegeneration. The CS of the two groups was also compared according to the logarithmic values measured at spatial frequencies of 1.5, 3, 6, 12 and 18 cycles per degree in photopic light using functional acuity contrast test (FACT). Results: Twenty-five eyes of 25 patients with insulin resistance (insulin resistant group) and 25 eyes of 25 healthy subjects (control group) were included in this study. There were no statistically significant differences between the two groups in any of the spatial frequencies in the FACT. The mean average GCIPL thickness and mean GCIPL thickness in the inferotemporal sector were significantly less in the insulin-resistant group when compared with the control group (mean average GCIPL thicknesses in the insulin-resistant and control groups were 83.6 +/- 4.7 mu m and 86.7 +/- 3.7 mu m respectively, p = 0.01; mean inferotemporal GCIPL thicknesses in the insulinresistant and control groups were 83 +/- 6.0 mu m and 86.7 +/- 4.6 mu m respectively, p = 0.02). Conclusion: Although it may not lead to functional visual impairment such as CS loss, the retinal neurodegeneration seen in diabetic patients may begin in the insulin resistance stage.Öğe Evaluation of affective temperament and anxiety-depression levels of patients with polycystic ovary syndrome(Elsevier Science Bv, 2015) Asik, Mehmet; Altinbas, Kursat; Eroglu, Mustafa; Karaahmet, Elif; Erbag, Gokhan; Ertekin, Hulya; Sen, HacerBackground: Women with polycystic ovary syndrome (PCOS) are reported to experience depressive episodes at a higher rate than healthy controls (HC). Affective temperament features are psychiatric markers that may help to predict and identify vulnerability to depression in women with PCOS. Our aim was to evaluate the affective temperaments of women with PCOS and to investigate the association with depression and anxiety levels and laboratory variables in comparison with HC. Methods: The study included 71 women with PCOS and 50 HC. Hormonal evaluations were performed for women with PCOS. Physical examination, clinical history, Hospital Anxiety and Depression Scale (DADS) and TEMPS -A were performed for all subjecLs. Differences between groups were evaluated using Student's [-tests and Mann Whitney U tests. Correlations and logistic regression tests were performed. Results: All temperament subtype scores, except hyperthyrnic, and DADS anxiety, depression, and total scores were significantly higher in patients with PCOS compared to HC. A statistically significant positive correlation was found between EMI and irritable temperament, and insulin and DADS depression scores in patients with PCOS. Additionally, hirsutism score and menstrual irregularity were correlated with DADS depression, anxiety and total scores in PCOS patients. In logistic regression analysis, depression was not affected by PCOS, hirsutism score or menstrual irregularity. However, DADS anxiety score was associated with hirsutism score. Conclusions: Our study is the first to evaluate the affective temperament features of women with PCOS. Consequently, establishing affective temperament properties for women with PCOS may help clinicians predict those patients with PCOS who are at risk for depressive and anxiety disorders. All rights reserved.Öğe Evaluation of circulating cell free DNA in plasma as a biomarker of different thyroid diseases(Assoc Brasileira Otorrinolaringologia & Cirurgia Cervicofacial, 2020) Caglar, Ozge; Cilgin, Begum; Eroglu, Mustafa; Cayir, AkinIntroduction: Many studies have been done on proteomics, genomics, epigenetic, immuno-genetics in many body fluids. Among these, circulating cell-free DNA (ccfDNA) entered the literature in 1948, but it has not been studied for many years due to technological deficiencies. Following recent advances, geno-metastasis has been mentioned and new research is needed in this area. ccfDNA is known to be an important biomolecule in this regard. Objective: The presence of cell-free DNA in the circulatory system may offer a tremendous opportunity to provide novel biomarkers for thyroid diseases. This experimental study was conducted to determine the amount of ccfDNA in different thyroid diseases, then to evaluate whether the ccfDNA concentration varied between the disease groups and control group. Methods: In total, we included 121 individuals in the present study. We collected blood samples and then determined the ccfDNA concentration in plasma of collected blood samples from three groups: thyroiditis (n=33), benign (n =37), and malignant (n =30) and from a control group (n=21). Results: The median values of the ccfDNA groups were found as 1610, 1665, 1685 and 576 ng/mL for the thyroiditis, benign, malign, and control groups, respectively. Findings showed that the ccfDNA of the three groups was significantly higher than the control (p <0.0001). Each group was compared in terms of ccfDNA and the p-values of benign-thyroiditis, benign-malign, and thyroiditis-malign were 0.09, 0.65, and 0.29, respectively. Conclusions: The clear differences between thyroid diseases and controls suggest that ccfDNA is worthy of attention as a biomarker for further evaluation of different thyroid diseases. However, EFT thickness in patients with hyperparathyroidism who were treated with parathyroidectomy has not yet been studied. In this study we aimed to examine whether carotid intima-media thickness (CIMT) and EFT thickness would be affected after treatment of PHPT by parathyroidectomy. Materials and methods: 27 patients with PHPT who were diagnosed with a parathyroidectomy indication and 39 healthy volunteers were included in the study. For all patients with PHPT, anthropometric measurements were made before and 12 months after parathyroidectomy, and EFT thickness, CIMT, serum parathormone (PTH), calcium and lipid levels were measured. Results: It was observed that preoperative CIMT and EFT thickness in patients with PHPT were significantly higher than the control group (for both p<0.001). Although hyperparathyroidism and hypercalcemia of the patients improved after parathyroidectomy, no significant change in the thickness of CIMT or EFT was determined. Conclusions: CIMT and EFT thickness seem to be good cardiovascular indicators for diagnosis of patients with PHPT. Nevertheless, this study raises the question of the reliability of a decrease in these parameters in patient follow-up. It is therefore necessary to be more careful in evaluating these parameters after treatment of patients with PHPT.Öğe EVALUATION OF THE RISK OF ATHEROSCLEROSIS IN PATIENTS WITH ADRENAL INCIDENTALOMA USING THE ULTRASOUND BIOMARKERS EFT AND CIMT(Carbone Editore, 2015) Sen, Hacer; Uysal, Fatma; Binnetoglu, Emine; Asik, Mehmet; Erbag, Gokhan; Gunes, Fahri; Eroglu, MustafaIntroduction: Adrenal incidentalomas (AIs) are masses frequently observed on radiological examination that have attracted attention due to their coincidence with metabolic disorders. Recently, the relationship between atherosclerosis and adrenal incidentalomas has been noted. Epicardial fat tissue (EFT) and carotid artery intima-media thickness (CIMT) measurements show the development of atherosclerosis (AS). The aim of this study is to evaluate the risk of cardiovascular disease in AI patients using EFT and CIMT readings. Materials and methods: Our study included 63 Al patients and 48 healthy controls. All patients were evaluated for blood pressure, Body Mass index(BMI), EFT, CIMT, and lipid parameters. Al patients were also investigated for hormonal secretion. The relationship between risk of atherosclerosis and EFT, CIMT was analyzed. Results: Control subjects (mean EFT, 3.92 +/- 1.14 mm) had lower EFT than patients with subclinical Cushing's syndrome (SCS) and non-functional adenoma (NFA) (mean EFT, 6.22 +/- 1.97 mm and 5.44 +/- 1.61 mm, respectively) (both P < 0.001). There was no difference in EFT between SCS and NFA patients. CIMT was significantly lower in control subjects (mean CIMT, 0.65 +/- 0.11 mm) than in NFA patients (mean CIMT, 0.75 +/- 0.18 mm) (P < 0.01). The EFT of SCS patients was not significantly different from the EFT of either NFA patients or controls. Ischemia-modified albumin (IMA) is a marker of ischemia and oxidative stress and is increased in different clinical conditions such as cardiovascular diseases and metabolic syndrome. However, it has not been investigated in the patients with SCS. We aimed to evaluate serum IMA levels in the patients with SCS. Materials and methods: A total of 128 patients with AI were included in this study (17 patients with SCS and 111 patients with non-functional adenomas (NFA)). All patients were evaluated for the presence of adrenal masses using adrenal computed tomography (CT) scans. Serum IMA levels were measured by using a colorimetric method. Results: Serum IMA levels were significantly higher in SCS patients than in NFA patients (p < 0.05). Serum IMA was significantly correlated with waist circumference, low-density lipoprotein (LDL) levels and SCS. Furthermore, regression analysis revealed that serum IMA levels are independent and positively associated only with SCS. Conclusion: We concluded that elevated serum IMA levels might be accepted as a useful marker in patients with SCS.
