Öğe Biochemical parameters in professional male footballers before and after training(Taylor and Francis Ltd., 2017) Colak, Serap; Duman, Can; Bamac, Belgin; Colak, Enis; Ozbek, Aydin; Colak, Tuncay; Sivri, IsmailThe goal of this study is exploring serum alterations related to acute coronary sendrom, muscle deterioration, hypoglysemia and the other variables in the serum of the fifteen footballers. Fifteen professional male footballers (mean (SD) age: 24 (4.4) years) were gathered from a 3rd league team. Blood samples were collected from them before and after exercise. Blood samples were analyzed by standard methods and the results were corrected in the biochemistry laboratory. The average pre-and post-exercise biochemical values of 15 footballers are found below. The glucose in the serum is 62.3 ± 3.9(before) and 69.5 ± 7.8(after) (P<0.05) for preand post-exercise, respectively. The urea in the serum is 36 ± 9.5(before) and 34.4 ± 9.2(after) (p>0.05). The creatine in the serum is 0.9 ± 0.l(before) and 0.8 ± 0.9(after) (p>0.05). The CK in the serum is 309.9 ± 200.9(before) and 253 ± 149.7(after) (p>0.05). The CK-MB in the serum is 17.9 ± 6.3(before) and 14.5 ± 5.4(after)(p>0.05). The total bilirubin in the serum is 0.8 ± 0.4(before) and 0.8 ± 0.5(after) (p>0.05). The albumin in the serum is 4.1 ± 0.3(before) and 4.1 ± 0.2(after) (p>0.05). The BUN (blood urea nitrogen) in the serum is 16.7 ± 4.4(before) and 16.3 ± 4.5(after) (p>0.05). Although broad variety of biochemical perturbations is present during acute physical stress, many parameters remain within regular limits. © Kamla-Raj 2017.Öğe Bir üniversite hastanesine başvuran sağlıklı insanlarda iskemi modifiye albuminin referans değerleri(2014) Duman, Can; Bakar, Coşkun; Eşsizoğlu, Ertan; Tütüncüler, Funda Kırtay; Demircan, ElifAmaç: İskemi modifiye albumin (IMA) akut koroner sendromun tanısında potansiyel kullanımı olabilecek bir miyokardiyal iskemi belirtecidir. Bu çalışmada amacımız Çanakkale Onsekiz Mart Üniversitesi'ne başvuran 15 yaşın üzerindeki sağlıklı insanlarda IMA testinin referans değerlerini belirlemektir.Gereç ve Yöntemler: Bu çalışma Çanakkale Onsekiz Mart Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesine başvuran 15 yaş ve üzeri ve görünürde sağlıklı 582 kişi üzerinde gerçekleştirilmiştir. Çalışma grubunun yaş ve cinsiyet dağılımı belirlenirken 2011 yılı Türkiye nüfusu referans olarak alınmıştır. Katılımcılara demografik özelliklerinin ve sağlık problemlerinin sorgulandığı bir anket formu uygulandı ve 9 ml venöz kan alınmıştır. IMA düzeyleri Bar-Or tarafından tanımlanan spektrofotometrik metot kullanılarak ölçülmüştür. IMA referans değerlerini saptamak için ortalama, standart sapma, ortanca, minimum, maksimum ve % 2,5 - % 97,5 persentil değeri kullanılmıştır.Bulgular: Toplamda 274 erkek (% 47,1) ve 308 kadın (% 52,9) değerlendirildi. Çalışılan grupta ortalama IMA değeri 0,339 ± 0,093, ortanca değeri 0,343 (Min-Maks: 0,053 - 0,631), % 2,5 - % 97,5 persentil değeri 0,145-0,530'dir. Sonuçlar: İskemik durumların tanısında IMA'nın referans değerlerini vurgulayan çalışmalar yetersizdir. Myocardial damage related to the cardiac ischemia is more prominent especially in the first 6 h. In the present study, circulatory erythropoietin levels in response to cardiac ischemia were evaluated. Materials and methods: Patients with stable angina who underwent balloon angioplasty (study group, n = 55) and hospitalized for coronary angiography (as control group, n = 23) were enrolled into the study. Serum erythropoietin levels were measured in both groups in baseline, 6 and 18 h after the procedure. Results: Coronary balloon inflation time was accepted as duration of myocardial ischemia. Study group showed significant erythropoietin reduction at sixth hour compared to control group. Erythropoietin reduction at sixth hour was significantly correlated with duration of myocardial ischemia. Conclusion: Decreased circulatory erythropoietin levels in the early phase of acute cardiac ischemia may accelerate the apoptotic activity. Recombinant erythropoietin replacement to prevent erythropoietin decrease following cardiac ischemia may have negative effect on myocyte loss. (C) 2008 Elsevier Ireland Ltd. As is known, however, use of pesticides takes a toll on humans, animals and the environment health-wise and insensible and intensive use causes residues in food, soil, water and air. Even though the use of pesticides creates an increase in quantity product-wise, they are potential toxic materials for humans and animals. In this study, New Zealand albino rabbits (75 days old, 2 kg average live weight) were given chlorsulfuron and vitamin C through feed for 45 days, their liver enzyme activities were monitored and it was checked whether or not the liver was damaged. In the research, levels of GGT and ALK.P in the serum samples received from trial group animals were observed to be sensible in comparison to the control group (p<0.05). © 2014 Academic Journals Inc.Öğe Helicobacter pylori Seropositivity in Patients with Acute Coronary Syndromes(Springer, 2009) Tamer, Gulden S.; Tengiz, Istemihan; Ercan, Ertugrul; Duman, Can; Alioglu, Emin; Turk, Ugur O.The aim of this study was to investigate the seroprevalence of Helicobacter pylori (HP) in patients with coronary atherosclerosis and acute coronary syndromes. We enrolled 152 patients (group I, 73 patients with acute coronary syndrome; group II, 79 patients with chronic stable angina) and 22 control subjects. An enzyme-linked immunosorbent assay for immunoglobulin (Ig) G test for HP diagnosis was used on all enrolled subjects. C-reactive protein (CRP) was also measured in all patients as an inflammatory marker. Seropositivity rates for HP were significantly higher in patients with coronary artery disease than in controls (80.2% versus 54.5%; P < 0.05). Although CRP level was significantly higher in group I than in group II (1.72 +/- A 2.89 mg/dl versus 0.53 +/- A 0.63 mg/dl, P < 0.0001), HP infection rates were similar between groups (86.3% versus 74.6%, P > 0.05). Infection with HP may influence atherogenesis through low-grade, persistent inflammatory stimulation. However, our data show evidence of increased systemic inflammation in patients with acute coronary syndrome, which was unrelated to an increased HP seropositivity.Öğe Ischemia modified albumin levels and its association with clinical follow-up in acute renal failure(2017) Uzun, Mehmet; Topaloğlu, Ömercan; Kurtulmuş, Yusuf; Turkon, Hakan; Duman, Can; Karakas, Burak; Akar, HarunAim: In cases with acute ischemia, albumin’s binding capacity for transition metals decreases and the resulting albumin is defined as ischemia modified albumin (IMA). In this study, we aimed to investigate the relationship between IMA and clinical follow-up in patients with acute renal failure (ARF).Material and Methods: Levels of IMA were measured in 51(23 male, 28 female) patients with ARF. Venous blood samples were drawn from patients for biochemical tests and put in plain tubes containing the gel.Results: Mean age of male and female patients was 65.39±15.28 and 70.11±15.25, respectively. The IMA levels in 25.5% of the patients were within the normal range (<400 ABSU), while the IMA levels were higher (>400 ABSU) in 75.5% of the patients. The survival rates of patients in IMA <400 ABSU group for 12 and 24 months were 66.7% and for 30 and 32 months it was 33.3%; while the survival rates of patients in IMA ?400 ABSU group for 12 months were 85.8%, for 24 months were 61.3%, and for 30 and 32 months were 30.6%. No significant difference was determined among survival rates of IMA groups (p=0.719).Conclusion: The comparison between the groups having normal or higher IMA values did not show any significant differences in terms of survival. However, in our study the proportion of patients who needed dialysis during treatment were significantly higher in higher IMA group(IMA?400 ABSU). Therefore, we believe that higher IMA levels may indicate a necessity for dialysis in patients with ARF.Öğe Ischemia-modified albumin levels in professional male soccer players before and after training(Marmara Univ, Fac Medicine, 2013) Duman, Can; Colak, Tuncay; Bamac, Belgin; Goker, Imran; Colak, Serap; Ozbek, AydinObjectives: The ischemia-modified albumin (IMA) test is a promising indicator of myocardial ischemia in the early diagnosis of the acute coronary syndrome (ACS). Recent studies examining alterations in IMA levels caused by myocardial ischemia that develops during exercise have shown contradictory results. There has been an increase in severe exercise-related cardiac events in football. We therefore, assessed IMA levels before and after exercise in professional soccer players in order to examine the effect of the exercise on IMA levels. Methods: Blood was collected from professional soccer players before and after exercise. IMA levels were measured by using spectrophotometric methods, and the albumin levels were measured in an autoanalyzer. Results: The average pre- and post exercise IMA levels of 16 soccer players were found to be 0.438 +/- 0.071 and 0.386 +/- 0.069, respectively. The pre-and post exercise albumin levels of the same group were 4.08 +/- 0.20 g/dL and 4.21 +/- 0.19 g/dL, respectively. There was a strong negative correlation between post exercise IMA and albumin levels, which was statistically significant (r = -0.535. p = 0.033). Conclusion: IMA levels should be examined not only in non-cardiac pathologies, but also in specific serum albumin concentrations and in individuals practicing vigorous sport activities.Öğe Ischemia-modified albumin use as a prognostic factor in coronary bypass surgery(Biomed Central Ltd, 2012) Kanko, Muhip; Yavuz, Sadan; Duman, Can; Hosten, Tulay; Oner, Emin; Berki, TuranBackground: Various types of markers have been used so far in order to reveal myocardial perfusion defect. However, these markers usually appear in the necrosis phase or in the late stage. Having been the focus of various investigations recently, ischemia-modified albumin (IMA) is helpful in establishing diagnosis in the early stages of ischemia, before necrosis develops. Methods and Results: 30 patients that underwent only coronary bypass surgery due to ischemic heart disease within a specific period of time have been included in the study. IMA levels were studied in the preoperative, intraoperative, and postoperative periods. The albumin cobalt binding assay was used for IMA determination. Hemodynamic parameters (atrial fibrillation, the need for inotropic support, ventricular arrhythmia) of the patients in the postoperative stage were evaluated. Intraoperative measurement values (mean +/- SD) of IMA (0.67677 +/- 0.09985) were statistically significantly higher than those in the preoperative (0.81516 +/- 0.08894) and postoperative (0.70477 +/- 0.07523) measurements. Considering atrial fibrillation and need for inotropics, a parallelism was detected with the levels of IMA. Conclusions: IMA is an early-rising marker of cardiac ischemia and enables providing a direction for the treatment at early phases.Öğe İskemi Modifiye Albuminin İnflamatuvar Parametrelerle İlişkisi(2013) Duman, Can; Eşsizoğlu, Ertan; Demircan, Elif; Kırtay, Funda Tütüncüler; Bakar, Coşkun; Kurtulmuş, YusufAmaç: İskemiye yol açan ateroskleroz ve artmış inşamatuvar durumlarda IMA ve CRP düzeyleri birlikte artar. Bu nedenle inşamatuvar durumu belirlemek için CRP ve sedimantasyon ölçümü yapılan hastalarda IMA düzeylerini saptayarak aralarındaki ilişkiyi inceledik. Gereç ve Yöntem: Rutin biyokimya analizleri için başvuran 117 hastanın CRP düzeyleri nefelometrik, sedimantasyon optik yöntemle, IMA absorbansı ise Bar-Orun spektrofotometrik metoduyla analiz edildi. Bulgular: Hastaların (44 erkek, 73 kadın) yaş ortalaması 50.07±18.02 yıldı. Ortalama IMA düzeyleri 0.37±0.08 absorbans ünitesi, CRP 1.15±2.66 mg/dL, Sedimantasyon 30 dk. 12.39±12.23 mm ve Sedimantasyon 60 dk. 28.34±21.12 mm olarak saptanmıştır. Cinsiyete göre yaş, IMA, CRP, Sedimantasyon 30 ve 60 dk. değerleri arasında anlamlı farklılık saptanmamıştır (p>0.05). IMA değeri ile CRP, Sedimantasyon 30 ve 60 dk. değerleri arasında anlamlı pozitif yönlü korelasyon saptanmıştır (p<0.05). IMA değeri ile yaş arasında anlamlı bir korelasyon bulunamamıştır. Sonuç: Sedimantasyon ve CRP gibi inşamatuvar parametrelerdeki artışın iskemi göstergesi olan IMAda artışla birlikte olduğunu belirledik. İskeminin yol açtığı oksidatif stresin CRP ve sedimantasyonu da arttıran proinşamatuvar reaksiyonları başlattığını düşünmekteyiz. We aimed to reveal the possible value of IMA in predicting the development of contrast induced nephropathy (CIN) after coronary angiography in patients with stable angina pectoris.Material and Methods: 106 patients underwent coronary angiography with a diagnosis of stable angina pectoris were included in our study. Basic demographic and clinical findings and laboratory values were recorded and analyzed. Serum creatinine (SCre) levels were also measured 48 hours after coronary angiography and recorded. Amount of contrast agent (CA) given during coronary angiography was recorded. The patients were divided into 2 groups: CIN positive and CIN negative groups.Results: CIN was developed in 14 patients (13%); and IMA levels were similar in CIN positive and negative groups (p>0.05). SCre (both measurements before and after CA administration) was not correlated with IMA levels. There was no association between drug usage and development of CIN (p>0.05). Comorbidities were not associated with the development of CIN (p>0.05) with the exception of hypertension (HT). Presence of hypertension (p=0.0393) and female gender (p=0.0199) was associated with development of CIN. Mean age was 61.3 and 52.3 in CIN positive and negative groups, respectively (p>0.05).Conclusion: Any specific biomarker indicating CIN is not available yet. Most frequently used marker is the measurement of SCre 24- 48 hours after administration of CA. We found IMA levels not to be a predictor for the development of CIN. Further investigations will clearly determine the importance of IMA as a biomarker in renal failure developed after CA administration.Öğe N-Acetylcysteine in Preventing Contrast-Induced Nephropathy Assessed by Cystatin C(Wiley-Hindawi, 2013) Alioglu, Emin; Saygi, Serkan; Turk, Ugur; Kirilmaz, Bahadir; Tuzun, Nurullah; Duman, Can; Tengiz, IstemihanAims: Prophylactic oral N-acetylcysteine (NAC) has been widely used for prevention of contrast-induced nephropathy (CIN). However, clinical studies have not been demonstrating this effect consistently because of evidence that NAC can alter serum creatinine levels without affecting glomerular filtration rate (GFR). We investigated NAC for the prevention of CIN by monitoring creatinine and cystatin C. Methods: We enrolled 113 patients (49 patients in NAC group and 64 patients in control group) with normal to subnormal GFR who were scheduled for cardiovascular procedures. Patients in NAC group receive acetylcysteine 600 mg twice a day, on the day before and on the day of cardiovascular procedure. All patients received a periprocedural intravenous infusion (volume expansion) of 1 ml/kg/h with 0.45% saline for 24 h (12 h before and 12 h after exposure to contrast medium). Serum cystatin C and creatinine levels were measured before and at 12, 24, and 48 h after procedure. Results: The incidence of cystatin C-based CIN was 28.5% (n = 14) in NAC and 23.4% (n = 15) in control group (p = 0.663) and serum creatinine-based CIN was 12.2% (n = 6) in NAC and 17.2% (n = 11) in control group (P= 0.468). In this study, oral NAC had no effect on the prevention of CIN in patients undergoing cardiovascular procedures. Conclusion: In this study, oral NAC administration does not reduce neither the incidence of cystatin C-based CIN nor serum creatinine-based CIN in patients undergoing cardiovascular procedures.Öğe The effect of anaesthesia technique on maternal and cord blood ischaemia-modified albumin levels during caesarean section: A randomized controlled study(Sage Publications Ltd, 2013) Omur, Dilek; Hacivelioglu, Servet Ozden; Oguzalp, Huseyin; Uyan, Berna; Kiraz, Hasan Ali; Duman, Can; Tutunculer, Funda KirtayObjective: Ischaemia-modified albumin (IMA) is an early marker for various ischaemic events, including cardiac ischaemia. This study determined variations in IMA levels during caesarean section, performed under general anaesthesia or with combined spinal epidural anaesthesia. Methods: Full-term, healthy pregnant women were allocated to undergo caesarean section, using either general anaesthesia or combined spinal epidural anaesthesia. IMA and albumin levels were measured in maternal serum samples taken immediately prior to caesarean section and 30min into the procedure, as well as from serum taken from cord blood after double clamping. Results: At total of 51 healthy pregnant women underwent either general anaesthesia (n=28) or combined spinal epidural anaesthesia (n=23). Within-group analysis of the general anaesthesia group showed that both IMA levels and IMA/albumin ratios were significantly higher at 30min of surgery compared with the immediate preoperative period. Conclusions: Lower IMA levels in the combined spinal epidural anaesthesia group may have been due to improved balancing of oxidative stress during caesarean section. The aim of this study was determine reference values of IMA test on the healthy people over the age of 15 who's admitted to canakkale Onsekiz Mart University. Methods: This study was performed on 582 people over 15 years old and healthy in appearance who had presented at the canakkale Onsekiz Mart University Faculty of Medicine Research and Application Hospital. The year 2011 population of Turkey was taken as the reference in the distribution of the study group according to age group and gender. A survey which queried the demographic features and health problems was administered to the participants of the study and 9 cc venous blood was drawn. IMA levels were measured by using the spectrophotometric method defined by Bar-Or. The mean, standard deviation, median, minimum, maximum, and 2.5 % - 97.5 % interval were used in order to detect the IMA reference values. Results: A total of 274 males (47.1 %) and 308 women (52.9 %) were studied. The mean IMA value of the examined group was 0.339 +/- 0.093, and the median was 0.343 (Min-Max: 0.053 - 0.631), while the 2.5 % - 97.5 % percentil values were 0.145 - 0.530. Conclusion: Studies that indicate the reference levels of the IMA for the diagnosis of ischemic conditions are insufficient. Our study is expected to contribute to closing this gap.
