Intravenous immunoglobulin therapy decreases the major cardiovascular complication of the disease. Atypical clinical conditions may accompany to the disease and these cases seem to be more prone to intravenous immunoglobulin resistance which is an important problem in the management of the cases. Herein, we presented a case of 4-year-old boy admitted to our emergency department with fever of 5 days duration and diagnosed as Kawasaki disease based on the clinical, laboratory and imaging findings. He was resistant to intravenous immunoglobulin treatment and in the follow up resulting abdominal tenderness was explained by gallbladder hydrops. In patients with Takayasu arteritis, hypertension is an important symptom which occurs as a result of narrowing of renal arteries and/or aorta. A 14-years-old girl was referred with facial flashing and tenderness on her face. Hypertension was found in upper limbs. Femoral pulses were weak and there was an abdominal murmur. On MR angiography, there was occlusion in aorta and renal arteries. The patient was diagnosed as TA, with increased acute phase reactants. After an antiinflammatory, an antihypertensive and immunsuppresive treatment, patient's clinical complaints and laboratory findings were under control. In this case, we want to emphasize the importance of blood pressure measurement and four limb pulse examination in childhood in routine physical assesment. In this article, the lipoma causing median nerveentrapment will be discussed, although it is notlocated just in the carpal tunnel but distally.Öğe Acil Servise Başvuran Hastalarda Pulmoner Bt Anjiografi Tetkikinin Tanı Verimliliğinin Değerlendirilmesi(2022) Ertem, Şenay Bengin; Aylanç, Nilüfer; Daş, Murat; Bardakçı, OkanAmaç: Acil servislerde ileri görüntüleme yöntemleri giderek artan oranlarda kullanılmaya başlamıştır. Bu hızlı artış beraberinde; hem söz konusu görüntüleme yöntemlerinin tanı verimliliğinin düşük olması olasılığını; hem de bu metotlara bağlı gelişebilecek olası zararları gündeme getirmiştir. Bu konuda acil servislerde pulmoner bilgisayarlı tomografi anjiografi (pulmoner BTA) kullanımı özellikle ön plana çıkmaktadır. Gereç ve Yöntemler: Çalışma, Ocak 2016 ile Haziran 2018 ayları arasında acil servise başvuran ve pulmoner emboli (PE) ön tanısı ile pulmoner BTA tetkiki istenen hastaların radyolojik görüntülerinin ve medikal kayıtlarının retrospektif olarak incelenmesi yöntemiyle yapıldı. Çalışma verileri SPSS 19 paket programı kullanılarak analiz edildi. Sayısal değişkenlerin karşılaştırmasında Mann Whitney U testi kategorik değişkenlerin karşılaştırmasında Ki-Kare testi kullanıldı. İstatistiksel verilerin yorumunda p<0,05 anlamlı olarak kabul edildi. Bulgular: Çalışma döneminde acil servise başvuran ve PE ön tanısı ile pulmoner BTA çekilen toplam 233 hasta tespit edildi. Hastaların 11’i artefaktlar nedeniyle çalışma dışında bırakıldılar. Geriye kalan 222 hastaya ait veriler analiz edildi. Hastaların 19’unda (%8,6) PE pozitif (Grup-1); 203 hasta (% 91,4) Pulmoner emboli negatif (Grup-2) olarak saptandı. Ayrıca PE tespit edilmeyen (Grup-2) hastalardan ikisinde (%0,9) herhangi bir patoloji izlenmedi ve normal olarak raporlandı. Her iki grupta da hastaların acil servise en sık nefes darlığı ikinci sıklıkla göğüs ağrısı, şikayetleri ile başvurdukları görüldü (Tablo-I). Sonuç: Sonuç olarak, PE ön tanısı ile istenen pulmoner BTA’nın tanısal verimliliğini, literatürle uyumlu olarak düşük (%8,6) bulduk. The risk of appendicitis increases in these patients since the blood supply to the appendix can be impaired. The condition is frequently asymptomatic, and even if symptomatic it gives rise to non-specific symptoms. There is no specific laboratory finding. All rights reserved.Öğe An Overlooked Posttraumatic Lesion: Morel Lavallee(2020) Aylanç, Nilüfer; Ertem, Şenay Bengin; Akyürek, MustafaMorel-Lavallee is a soft tissue lesion that occurs after trauma and is seen almost rare. The patients usually refer to the clinic with a palpable mass. In addition, lesions can be detected incidentally during imaging tests performed for different reasons after trauma. While examining the soft tissue lesions of the musculoskeletal system, especially in cases with a history of trauma, the Morel-Lavallee lesion should also be thought, and ultrasonography and magnetic resonance imaging methods that have superiority in soft tissue imaging should be used. In this case report, we aim to attract attention to the fact that MLL, which can be seen rarely, should be kept in mind in cases with pain, palpable mass and a trauma history, even if the process is chronic.Öğe Comparison of lung ultrasonography, transthoracic echocardiography and clinical findings in assessing volume status in patients receiving renal replacement therapy(2024) Baştürk, Gizem; Bakırdogen, Serkan; Küçük, Uğur; Aylanç, Nilüfer; Çelik, Zübeyir; Basturk, GökhanWhile transthoracic echocardiography and clinical findings are used to assess body volume status, lung ultrasonography (LUS), which is a non-invasive method, has also been used in assessing hypervolemia in recent years. This study aims to compare LUS, transthoracic echocardiography (TTE) and clinical findings in the assessment of body volume status in patients receiving renal replacement therapy (RRT). The study included 99 subjects who received RRT and were followed-up in our hospital. The patients were randomly selected from kidney transplant patients and those who had been receiving peritoneal dialysis (PD) or hemodialysis (HD) for at least the past three months. In addition, a control group consisting of 52 healthy subjects was also enrolled. LUS and TTE were simultaneously performed (within 24 hours). The mean left atrial (LA) diameter was 37.8±2.7 mm, 37.9±3.4 mm, 38.6±3.5 mm and 29.0±4.4 mm in patients who received PD, kidney transplantation, HD and in the control group, respectively. There was a moderate positive correlation between the total number of B-lines and LA diameter (mm) in the combined group consisting of the patient and control groups (p=0.0001, r=0.286). In this study, the number of B-lines on LUS has been found to be higher in patients receiving renal replacement therapy due to the effect of hypervolemia compared to the control group. However, there was no statistically significant difference in terms of the number of B-lines and LA diameter in echocardiography.Öğe Could radio-morphological changes in humerus greater tuberosity be an early indicator of rotator cuff tendon injuries?(Springer Science and Business Media Deutschland GmbH, 2022) Aylanç, Nilüfer; Ertem, Şenay BenginObjectives: To reveal the possible relationship between greater tuberosity volume with rotator cuff tendon injuries. Materials and methods: In our study, the morphology of the greater tuberosity was evaluated retrospectively by using radiographs and MR images of 335 cases. While critical shoulder angle (CSA) was measured on radiographs, greater tuberosity (GT) area and volume were calculated, and also supraspinatus, infraspinatus, and teres minor tendons were evaluated, on MR images. The relationship of the measurements with the tendon injuries was analyzed statistically. Results: Among 188 female and 147 male patients, no statistically significant relationship was found between tendon injuries in terms of gender. The relationships between supra and infraspinatus tendon injuries and GT angle, area, and volume measurements were statistically significant. There was a statistically significant relationship between teres minor injury and GT angle, but no significant correlation with GT area and volume. Finally, there was no statistically significant correlation between all three tendon pathologies and CSA. Conclusion: Area, and especially volume measurements, which are morphological features of the GT, may reflect tendon damage without significant degenerative changes in the bone structure. In examinations before tendon damage progresses, GT volume is a morphological feature that needs to be evaluated and may facilitate early recognition of tendinopathies and be an early marker of rotator cuff injuries.Öğe Dermatomyositis complicated case with muscular hematoma(Çanakkale Onsekiz Mart Üniversitesi, 2014) Aylanç, Nilüfer; Reşorlu, Mustafa; Adam, Gürhan; Karatağ, Ozan; Uysal, Fatma; Öğretmen, ZerrinA 62 years old woman diagnosed with dermatomyositis went to MRI examination because of her left hip pain and on her imaging findings the high signal intensities on iliopsoas, proximal thigh muscles and some pelvic muscles we determined, in addition there was no any contrast enhancement. However, there are haemorrhages on her left psoas muscle. We evaluated these findings as haemorrhagic myositis, which is the rare situation seen during follow up such these patients.Öğe Diagnostic agreement of pediatricians and radiologists on chest x-rays in pediatric emergency department Diagnostic agreement of pediatricians and radiologists on chest x-rays(Bayrakol Medical Publishing, 2021) Aylanç, Nilüfer; Battal, Fatih; Güzel, Yusuf; Öz, KorayAim: In this study, we aimed to investigate the diagnostic agreement on chest x-rays between pediatrician and radiologist in anatomical and pathological evaluation.Material and Methods: Chest radiographs of 700 cases from the pediatric emergency clinic were included in the study. The radiographs were evaluated by the pediatrician and radiologist as double-blind from a technical point of view (posture-position, inspiration adequacy, X-ray dose) and interpretation (hilar, mediastinal, parenchymal pathologies and cardiothoracic ratio). For results, Kappa analysis was used.Results: The mean age of the patients was 4.5 ± 4.5 (min 0-max 18) years, 335 (47.9%) were females and 365 (52.1%) were males. The diagnostic agreement was found as moderate level with adequate inspiration, optimal position, mediastinal width, and left hilar fullness; low level of disagreement was found in optimal dose, pleural fluid, consolidation, mediastinal fullness, right hilar fullness, infiltration, peribronchial thickening, reticular signs and very good agreement was found in CTR. It was statistically significant in terms of all parameters (p <0.001). Discussion: There may be differences, especially between interpretations of radiological examinations. To minimize these differences and to establish an accu-rate and precise diagnosis, the clinician and radiologist should have close contact. Compliance and consensus in the evaluation in the radiological assessment will help to manage the treatment process precisely and correctly.Öğe Diagnostic Inexperience of Takayasu Arteritis in Pediatric Neurology: A Case Report and Mini-Review of the Literature(2023) Çokyaman, Turgay; Sınmaz, Yücel; Aylanç, Nilüfer; Akıncı, Ayşe Burcu; Sözeri, BetülTakayasu arteritis (TA) is a chronic inflammatory vasculitis involving the aorta and its main branches. It usually starts with systemic inflammatory signs after ten years of age. Neurological symptoms seen depend on aneurysmatic, stenotic and thromboembolic events in the affected vessels. It is rarely seen in childhood and presentation with epileptic seizure is extremely rare in infantile age. In this case report, a 22-month-old child who was admitted with epileptic seizure and had a large infarction in the area matching the right middle cerebral artery (MCA) watershed. Symptoms and imaging findings due to infarction developed hours after epileptic seizure. First, low molecular weight heparin treatment was started. Following the development of multiple aneurysmatic-stenotic lesions in the left brachial artery and profunda branch, diagnosed as TA. It was added to oral steroid and azathioprine. Resistant seizures were controlled with levetiracetam and valproic acid in the poststroke period. Multidisciplinary follow-up is ongoing with anticoagulant, antiepileptic and immunosuppressive treatments. TA rarely occurs in the infantile period with acute neurological symptoms such as epileptic seizure and stroke. Bu çalışma, sistemik inflamasyonunbiyobelirteçlerini kullanarak KOAH'ın heterojenliğiniaraştırmak için yapılmıştır.Gereç ve Yöntem: 19 KOAH'lı hastanın toraks bilgisayar tomografisi amfizem baskın ve kronik bronşit baskın olmasına göre değerlendirildi ve kanbiyobelirteçleriyle karşılaştırıldı.Bulgular: Çalışma grubunun yaş ortalaması 72,1±8,0yıldı. KOAH fenotipiyle kan biyobelirteçleri arasında birfark saptanmazken, amfizem baskın ile kronik bronşitarasında, toraks anteroposterior (AP) ve transvers çapaçısından istatistiksel olarak anlamlı fark saptanmıştır.AP çap/transvers çap oranı ile TNF-alfa arasında pozitifyönlü güçlü istatistiksel olarak anlamlı korelasyon saptandı.Sonuç: Yaygın kronik bronşitli hastalar ile yaygınpulmoner amfizemi olan hastalar arasında inflamatuarbiyobelirteçler açısından anlamlı fark bulunmaması, buiki klinik ve fonksiyonel fenotipin benzer inflamatuarbir paterne sahip olabileceğini düşündürmüştür.Sonuçolarak, KOAH'ın fenotipik ayrımı ya da tedaviye yanıtındeğerlendirilmesi için biyobelirteçlerin kullanımınabüyük bir ilgi vardır. Sonuçlarımızın, daha büyük klinikçalışmaların planlanmasında fayda sağlayacağını düşünüyoruz.Öğe Patellar tendon ganglion cyst with Osgood Schlatter disease: a rare coexistence(Kuwait Medical Association, 2021) Aylanç, Nilüfer; Eker, Nebil; Reşorlu, MustafaPatellar tendinopathy is one of the causes of anterior knee pain. Osgood Schlatter disease (OSD) is another reason for pain in this location, occasionally accompaning patellar tendinopathy. We present magnetic resonance imaging and radiographic findings in a case of OSD with incidentally diagnosed patellar intratendinous ganglion that is a rare coexistence. We report a case of a 30-year-old male who presented with anterior knee pain, prolonged for 20 years. In radiologic evaluation, there was opacity in the patellar tendon and there was osseous fragment near the tibial tuberosity on lateral radiographic image. Except for this lesion, patellar tendon had the signal intensity changes indicating patellar tendinopathy. Besides these findings, there was bone irregularities and bone marrow edema on the tibial tuberosity and osseous fragment neighboring these structures with surrounding soft tissue edema. Patellar tendinopathy is a pathophysiological process that can be seen in people with knee pain and can occur due to recurrent trauma. Radiological findings can be encountered in a wide range from mild structural changes in the tendon due to degeneration to intratendinous lesions and adjacent bone pathologies such as OSD. Therefore, there is a relationship between two pathologies and magnetic resonance imaging plays an important role in the radiological diagnosis of these two entities.Öğe Persistan Sol Süperior Vena Kava: Nadir Bir Olgu Sunumu(2016) Reşorlu, Mustafa; Aylanç, Nilüfer; Adam, Gürhan; Özcan, Sedat; Eker, NebilPersistan sol süperior vena kava, genel popülasyonda %0,3-0,5 prevalansa sahip nadir görülen vasküler bir anomalidir. Sol anterior kardinal venin obliterasyonundaki yetersizlik sonucunda meydana gelmektedir ve konjenital kalp hastalığı olanlarda sıklığı artmaktadır. Bu olgularda aynı zamanda sağ süperior vena kava da sıklıkla mevcuttur. Persistan sol süperior vena kava tanısı genellikle kardiyovasküler görüntüleme sırasında insidental olarak konulmaktadır.Öğe Radiological Assessment of Dysbaric Osteonecrosis as a Part of Fitness-to-dive Examinations(İstanbul Training and Research Hospital, 2021) Korpınar, Şefika; Aylanç, Nilüfer; Ertem, Şenay Bengin; Reşorlu, MustafaIntroduction: It is important for professional divers and candidates to be medically and physically fit to undertake their underwater work safely. This study aimed to review the screening of dysbaric osteonecrosis (DON) using conventional radiography, which has been stated in the legislation as part of fitness-to-dive examinations, in terms of international standards and scientific facts. Methods: Data on clinical, demographic, laboratory, and radiological evaluations were retrospectively collected for 46 professional divers or candidates who were referred for fitnessto-dive examinations between 2018 and 2020. The approach of the Professional Divers Regulation to DON was compared with that of the Military Health Regulation for the Turkish Armed Forces and the Turkish Underwater Sports Federation Equipped Diving Instruction, as well as that of the legislation of United Kingdom and European Diving Technology Committee standards. Results: Forty-six divers (males: n=45; female: n=1) were enrolled in the study. Their median age was 34 (range: 18-63) years, and the mean body mass index was 25.42 kg/m2 . The prevalence of DON was 2.17% in the study group. Compared with the national and international legislations, Professional Diver Regulation was the only regulation that mandated radiological evaluation for DON in both the initial and periodic fitness-to-dive examinations. Conclusion: One of the aims of regulations is that medical fitness-to-dive standards should be evidence-based and compatible with contemporaneous clinical practice. By the 2000s, attitudes toward DON, fitness to continue diving, and compressed air work have changed in Europe. In light of the findings of the present study, we believe that it is time for a critical reappraisal of the Professional Divers Regulation.Öğe Radiological evaluation of anatomical changes in implant and adjacent segments after rigid fusion(2020) Malçok, Ümit Ali; Aylanç, NilüferAim: We aimed to determine the anatomical remodeling seen after lumbar spine stabilization in detail. Using magnetic resonanceimages, it is also aimed to reveal this remodeling is not only limited to the adjacent segment and also happens in the upperinstrumented vertebra region. At the end of this study, it is suggested to develop new radiological parameters to predict the changesin the adjacent segment and upper instrumented vertebra regions.Materials and Methods: Twenty cases operated for degenerative lumbar stenosis were included in our study. Quantitative datawere obtained by radiological measurements by a radiologist and neurosurgeon. On magnetic resonance images, the anatomicalstructures changed by remodeling were compared before and after the operation during (6-26 months) postoperative period. Unlikeprevious studies, anterior, middle and posterior disc heights; Cobb angle in the adjacent segment, spinal canal area, bilateral neuralforamen, facet joint areas and flavum thicknesses were evaluated.Results: From the sixth month after lumbar fusion, it was observed that angles and anatomical structures were changed in adjacentsegment. Also, it was observed that the areas with neural structures expanded in the upper instrumented vertebra region. Thedegeneration in adjacent segment and relaxation in the upper instrumented vertebra region were found to be statistically significant.A statistically strong positive correlation was found between the number of vertebrae included in the lumbar fusion and the meanheight of the adjacent segment disc (r: 0.526, p: 0.017).Conclusion: After comparing the measurements in adjacent segment and upper instrumented vertebra regions before and after theoperation, it was concluded that the remodeling was statistically significant. We suggest that the parameters in our study can beused as a scoring method for early detection of adjacent segment degeneration and/or disease. Juvenile type is a rare form that accounts for 5% of granulosa cell tumors. Isosexual precocious puberty is the most common presentation in prepubertal girls with juvenile granulosa cell tumors. Less frequently, they present with the complaint of a mass in the abdomen. We present an 8-year-old girl with a juvenile granulosa cell tumor who presented with sudden onset of abdominal pain and enlargement of the abdomen. She underwent exploratory laparotomy with right salpingo-oophorectomy and a complete mass resection was performed. Pathologic staging of the mass, which was diagnosed as juvenile granulosa cell tumor of the ovary, was determined as pT1c3, and our patient received four cycles of chemotherapy.
