(Sage Publications Inc, 2023) Alkan, Sevil; Sahinoglu, Mustafa Serhat
Buruli ulcer is caused by Mycobacterium ulcerans, a skin infection that occurs mostly in people living in the developing economies of Africa and is considered a neglected tropical disease by the World Health Organization (WHO). Left untreated, it can lead to chronic wounds and loss of limbs. This disease is one of the target diseases of the WHO, and there are very limited bibliometric studies published on this subject. Also, no similar study using the Web of Science Core Collection was found in the available literature. The aim of this study was to evaluate the bibliometric analysis of the literature on Buruli ulcers. For data visualization and analysis, the open-source visualization program Biblioshiny (version 2.0) was used. Although most publications are from Ghana, the United States, and European countries have also made significant contributions. The number of publications has increased especially since 2016. The most preferred keywords in the publications were treatment, diagnosis, and transmission routes. This is the first bibliometric analysis that examines the trend of scientific publications on Buruli ulcer that have been indexed in the Web of Science. Our findings have the potential to be used by academics to improve their research.

A case of acalculous cholecystitis or Brucella-induced acute cholecystitis?
(2023) Urkmez, Fatma Yekta; Sahınoglu, Serhat; Alkan, Sevil
Brucella spp. is a microorganism that can cause febrile, systemic infection, especially in endemic areas. Although it causes a multisystemic involvement and creates various clinical symptoms, osteoarticular symptoms are the most common. However, Brucella spp. is known to be a great mimic and can cause rare, atypical presentations. In this case report, we aimed to present a 67-year-old Turkish female patient diagnosed with stony cholecystitis due to Brucella spp. Although rare, brucellosis can be encountered as a cause of acute cholecyst. Although there are changes in the epidemiology of brucellosis, it is useful to keep in mind brucellosis in the differential diagnosis of many diseases, in endemic areas, especially in rural areas.

A case of acute brucellosis associated with HIV infection in Turkey
(Termedia Publishing House Ltd, 2024) Urkmez, Fatma Yekta; Alkan, Sevil
Human brucellosis is a common public health problem in Turkey. Human immunodeficiency virus (HIV)-positive patients are vulnerable to co-infections and super-infections due to targeting the immune system. Because opportunistic infections are considered primarily, it is very difficult to detect co-infections, especially Brucella spp., in this patient group. In addition, non-specific clinical findings of brucellosis make diagnosis difficult. In this report, an HIV-positive patient with acute brucellosis was described. A 28-year-old male patient receiving antiretroviral therapy was admitted to the hospital with complaints of high fever and myalgia for the last two weeks. Physical examination revealed enlargement of bilateral cervical lymph nodes and enlargement of bilateral inguinal lymph nodes. Primarily, opportunistic infections were investigated in this patient. Brucella spp. was detected in blood cultures taken due to high fever. Subsequently, the patient was diagnosed with brucellosis. We presented this case report with the purpose of drawing more attention to the clinical diagnosis of brucellosis. In addition, we recommend that clinicians should be more cautious about fever of unknown origin in HIV-positive patients, especially in regions where brucellosis is endemic. In the presence of newly developing fever and lymphadenopathy in HIV-infected individuals, opportunistic infections as well as other infectious diseases, including brucellosis, should be considered.

(Galenos Publ House, 2024) Bulbul, Emre; Evlice, Oguz; Ilki, Fahri Yavuz; Dindar, Emine Kubra; Ustun, Fatih; Sevinc, Ahmet Halil; Alkan, Sevil
Objective: There are insufficient data in the literature concerning whether cellular changes in tissue are reflected in blood circulation in granulomatous infections, such as zoonotic brucellosis. In this study, we compared laboratory parameters between patients with brucella (BEO) and non-brucella epididymo-orchitis (NBEO). Materials and Methods: This retrospective study included 84 patients with BEO and 92 with NBEO who presented to six medical centers between 2017 and 2021. Results: The median age of the patients was 41 (interquartile range: 27-61) years. In the multivariate analysis, the presence of abdominal pain (p=0.003), the percentage of lymphocytes (p=0.012) and the percentage of monocytes (p=0.029) were significantly higher in the BEO group than in the NBEO group. In addition, the percentage of neutrophils (p=0.001) was significantly lower in the BEO group than in the NBEO group. In the receiver operating characteristic analysis, the percentage of lymphocytes had an area under the curve (AUC) value of 0.808 at a cut-off point of >22.1%, the percentage of monocytes had an AUC value of 0.745 at a cut-off point of >7.7%, and the percentage of neutrophils had an AUC value of 0.746 at a cut-off point of <66.8%. Conclusion: To the best of our knowledge, there is no other study comparing the percentages of lymphocytes, monocytes, and neutrophils between patients with BEO and NBEO. The percentages of mononuclear leukocytes (monocytes and lymphocytes) and neutrophils may be useful for the prediagnosis of BEO in endemic areas.
