Öğe A Comprehensive Bibliometric Analysis on Neuronavigation Researches(Pera Yayincilik Hizmetleri, 2023) Akar, AliObjective: Neuronavigation is a novel method that has made great advances in the field of neurosurgery. The aim of this study was to review the published literature on this topic and to investigate the current state of research and trending topics. Methods: A bibliometric analysis of the neuronavigation on the Web of Science database was performed. The publications included in the study were analyzed with Bibliosiny, bibliometrix (version R 4.2.2), and the Literature Metrology Online Analysis Platform. Graphs from Web of Science were also utilized and some tables were created with the help of Excel. Results: The current study included 3026 research articles on neuronavigation. 11040 authors, 2332 affiliations, and 93 countries contributed to the literature on neuronavigation. The earliest publications on neuronavigation were published in early 1990. The first article was published in Germany. The international co-authorship rate was 17.78%. Most of the international co-authorship was between the United States and Germany. Conclusion: Neuronavigation research will remain a hot topic as technology advances. As a result, this study presented the trend and characteristics of neuronavigation studies, offering researchers a useful bibliometric analysis for future research.Öğe A visual and bibliometric analysis of Parkinson’s disease biomarker research(2023) Cinpolat, Havva Yasemin; Akar, AliAim: Biomarker research in Parkinson’s lacks a systematic bibliometric review, despite a growing body of bibliometric studies. This study aimed to analyze publications on biomarkers that may play an important role in the diagnosis and treatment of Parkinson’s disease. Materials and Methods: In the study, bibliometric methods were used to analyze the related research in depth. Data were obtained from the Web of Science Core Collection database. Specific keywords consisting of MeSH terms were used as search criteria. The search was conducted in English and the type of publication was selected as article. Microsoft Excel, VOS viewer and the visualization and analytical capabilities of the Biblioshiny R package were used in the analysis, providing a versatile approach to achieve the study objectives. Results: A total of 461 articles on Parkinson’s disease biomarkers published between 2002 and 2023 in 170 journals met the criteria and were analyzed. Regarding publications and citations, the year 2022 stood out. Each document had an average of 6.28 co-authors. The main contributing countries were the United States, China and Italy, with a global co-authorship rate of 30.8%. Notable institutions included the University of Washington, Ruhr University Bochum and the Paracelsus Elena Clinic. \"Journal of Neurochemistry,\" was the journal with the most publications. Through their total link strength, prominent terms such as \"Parkinson’s disease\", \"biomarker\" and \"alpha-synuclein\" showed significant occurrences and thematic relevance. Conclusion: The present study presents the inaugural bibliometric evaluation of biomarkers in Parkinson’s disease. The gradual increase of papers since 2012 indicates persistent scholarly interest and foretells continued significance over the following ten years. Future research will be aided by the study’s depiction of biomarker patterns, which provides insightful information. The term \"Alpha-synuclein\" stood out, highlighting its relevance in our research.Öğe BIBLIOMETRIC ANALYSIS ON DEEP BRAIN STIMULATION PROCEDURES CONDUCTED BETWEEN 2000-2023(Galenos Publ House, 2024) Cakir, Murteza; Akar, AliObjective: Deep brain stimulation (DBS) is a treatment method that uses electrodes to stimulate the brain, affecting brain activity and potentially causing medical conditions. It is commonly used to treat Parkinson's disease, essential tremor, dystonia, epilepsy, Tourette syndrome, and obsessive-compulsive disorder. This bibliometric study examines advanced bibliometric parameters in articles published since 2000, focusing on trends in publishing organizations, countries, funding sources, international collaborations, and trend keywords. Materials and Methods: We have searched the Web of Science database to find articles on DBS which published since 2000. The search was performed by using the MESH keywords releated to Deep brain stimulation. Results: This study presents a comprehensive analysis of 4,601 articles on Deep Brain Stimulation (DBS) from 2000 to 2023, focusing on publication trends, properties, funding, country contributions, and international collaborations. Noteworthy findings include a peak of 413 publications in 2020 and 14,992 citations in 2021. The overall trajectory demonstrates a significant increase in scientific output, with an average of 31.9 citations per article. Publication properties reveal diverse access categories, including 66 Early Access and 2,136 Open Access articles. The majority of records are in the Science Citation Index Expanded (93.980%). Clinical Neurology dominates research topics with 63.464% representation. Funding sources highlight major contributions from the USA, Germany, and China. The USA leads in research output, while the University of Toronto tops institutions. Major journals include Stereotactic and Functional Neurosurgery and Movement Disorders. Keyword analysis emphasizes common themes like deep brain stimulation and Parkinson's disease. International collaborations involve researchers from 75 countries, with the USA leading in total link strength. This study contributes valuable insights into the global landscape of DBS research. Conclusion: This analysis highlights the dynamic nature of Deep Brain Stimulation research, highlighting global collaboration and diverse topics, emphasizing the crucial role of key countries, institutions, and journals.Öğe Distribution of Lumbar Microdiscectomy Cases by Tuffier’s Line Levels(2023) Akar, Ali; Malçok, Ümit AliObjective: The most used method in the surgical treatment of lumbar disc herniation (LDH) is lumbar microdiscectomy (LMD). In this study, we aimed to show the relationship between Tuffier's line (TL), which is widely used in determining the L4-5 distance in lumbar spinal interventions, and LMD levels. Materials and Methods: In this study, a total of 204 patients who had LMD or had low back pain were reviewed retrospectively. According to the projection of TL at the spine level; it was divided into 5 categories as L4 corpus, L4 lower endplate, L4-5 disc space, L5 upper endplate, L5 corpus. The cases were analyzed according to age, gender, disc level and changes in TL. Results: 92.1% of the cases were operated at the L4-5 or L5-S1 level. TL was mostly at L5 upper endplate level in women, L4-5 disc space and above in men, and there was a significant relationship between TLs and gender in both groups (p<0.05). Conclusion: In our study, we found that although TLs were caudally in patients operated at L5-S1 level, they displaced cranially in patients operated at L4-5 level. However, it was observed that LDH development in the lower lumbar region and increased at the upper levels with ageing.Öğe EFFECTS OF LUMBAR MICRODISCECTOMY ON SPINOPELVIC PARAMETERS(Turkish Spine Society, 2021) Malçok, Ümit Ali; Akar, AliObjective: One of the causes which accelerates spinal degeneration process is lumbar disc hernia (LDH). In this study, we observed the changes in spinopelvic parameters one month after single-level lumbar microdiscectomy (LM). Materials and Methods: Standardized bilateral standing scoliosis radiography images of 19 patients, which were performed before and 1 month after LM operation, were evaluated retrospectively. Sagittal vertical axis and coronal vertical axis distances, thoracic kyphosis, lumbar lordosis, pelvic incidence, sacral slope and pelvic tilt angles were measured with Surgimap software. Results: There was a positive correlation between pre-and post-LM values of all spinopelvic parameters included in the study. In comparative tests however, the difference was found to be not statistically significant. As a result, despite LM operation slightly improved spinopelvic parameters in the first month, there was not a significant change overall. Conclusion: LDH is known to worsen the degenerative process in spine. LM as a treatment option is seen to alleviate this degenerative process by slowing down the deterioration in spinopelvic parameters.Öğe Neuroprotective Effects of Piceatannol on Olfactory Bulb Injury after Subarachnoid Hemorrhage(Springer, 2023) Akar, Ali; Öztopuz, Rahime Özlem; Büyük, Başak; Ovalı, Mehmet Akif; Aykora, Damla; Malçok, Ümit AliSubarachnoid hemorrhage (SAH) accounts for 5% of all stroke cases and is responsible for significant permanent brain and neurological damage within the first few days. Loss of smell is one of those neurological disorders following olfactory bulb injury after SAH. Olfaction plays a critical role in several aspects of life. The primary underlying mechanism of olfactory bulb (OB) injury and loss of smell after SAH remains unknown. Piceatannol (PIC), a natural stilbene, possesses anti-inflammatory and anti-apoptotic effects against various diseases. In this study, we aimed to investigate the potential therapeutic effects of PIC on OB injury following SAH at molecular mechanism based on SIRT1, inflammatory (TNF-α, IL1-β, NF-κB, IL–6, TLR4), and apoptosis (p53, Bax, Bcl-2, caspase-3)-related gene expression markers and histopathology level; 27 male Wistar Albino rats were used in a pre-chiasmatic subarachnoid hemorrhage model. Animals were divided into groups (n = 9): SHAM, SAH, and PIC. Garcia’s neurological examination, brain water content, RT-PCR, histopathology, and TUNEL analyses were performed in all experimental groups with OB samples. Our results indicated that PIC administration significantly suppressed inflammatory molecules (TNF-α, IL–6, IL1-β, TLR4, NF-κB, SIRT1) and apoptotic molecules (caspase-3, p53, Bax). We also evaluated edema levels and cell damage in OB injury after SAH. Ameliorative effects of PIC are also observed at the histopathology level. Garcia’s neurological score test performed a neurological assessment. This study is the first to demonstrate the neuroprotective effects of PIC on OB injury after SAH. Bu çalışma ile OPVK hastaların, osteoporoz (OP) düzeyleri ve tedavi alışkanlıkları ile OPVK nedenleri ve tedavi şekilleri arasındaki ilişkiyi ortaya koymayı amaçladık. Araçlar ve Yöntem: OPVK nedeni tedavi edilen 50 yaş üzeri 65 olgu retrospektif olarak incelendi. Hastaların müracaatları esnasında OP tanısı (var-yok) ve tedavileri (var-yok) olup olmadığına göre ayrıldı. OPVK tedavisi esnasındaki DEXA sonuçlarına göre hastalar osteopeni (T skoru -1 ve -2.5 arası) ve OP (T skoru<-2.5) olarak iki gruba ayrıldı. OPVK nedenlerine göre trafik kazası, düşme ve travma yok ve tedavi şekillerine göre medikal, kifoplasti ve posterior stabilizasyon olarak 3 gruba ayrıldı. Bulgular: Hastaların OPVK nedeni ile kliniğimize müracaatı esnasında hiçbirinde OP tanısının olmadığı, dolayısı ile OP medikal tedavisi almadığı görüldü. Kadın ve erkek cinsiyet arasında DEXA değerlerinde anlamlı bir fark olduğu tespit edildi (p=0.045). Yaş kategorisi ve OPVK nedenleri ile DEXA değerlerinin arasında anlamlı bir ilişki yoktu (p=0.144, p=0.163). Sonuç: Osteoporoz ve OPVK yaşam süresi uzadıkça giderek daha büyük ve daha önemli bir halk sağlığı sorunu haline gelmektedir. OPVK’dan korunmak için, OP’nin erken tanı ve tedavisi önemlidir. OP ve tarama yöntemleri için farkındalığın arttırılması gerekmektedir.Öğe Rarely Seen Intradiploic Skull Hematoma in Infants(Lippincott, Williams & Wilkins, 2021) Malçok, Ümit Ali; Akar, AliAlthough the first intradiploic hematoma (IH) case was reported in 1934, only 7 infant cases were reported in the literature. The authors present 2 IH cases, without birth trauma or bleeding disorder, which were treated surgically. IH is also included in the differential diagnosis of babies with a skull deformity. Computed tomography and magnetic resonance imaging modalities should be used together in diagnosis and treatment. The authors believe that the surgery to treat skull deformity because of IH in babies should be planned in the early period.Öğe THE UTILITY OF HEMATOLOGICAL INDICES AT THE INITIAL ADMISSION TO THE NEUROSURGERY CLINIC IN CERVICAL DISC HERNIATION(Galenos Publ House, 2024) Cinpolat, Havva Yasemin; Akar, Ali; Malcok, Umit Ali; Alkan, SevilObjective: This study aimed to investigate hematological indices to predict spontaneous regression in patients with cervical disc herniation (CDH) during the initial visit to outpatient clinics. Materials and Methods: This retrospective study was carried out at a single center by reviewing laboratory parameters to assess the outcomes of CDH patients. The cohort consisted of patients with CDH who had undergone surgery, those who had undergone conservative treatment and achieved spontaneous regression, and a control group without CDH. The laboratory data consisted of the neutrophil-tolymphocyte ratio (NLR), monocyte-to-lymphocyte ratio (MLR), and systemic immune-inflammatory index (SIII). Results: Differences in the NLR, MLR, and SIII were statistically significant across groups (p<0.001). Compared with the spontaneous regression and control groups, the surgical intervention group presented significantly greater NLR, MLR, and SIII values. According to the comparison of the patients with CDH in terms of the level of herniation, there were no significant differences at the C4-C5 and upper levels, whereas there was a statistically significant increase in the NLR, MLR, and SIII in the surgical intervention group compared with the spontaneous regression group at the C6-C7 level (p = 0.015, p<0.001, and p = 0.003, respectively). Conclusion: This study provides valuable insights into the use of hematological indices to predict the need for surgical intervention in CDH patients. The observed associations emphasize their practical use, providing a way for further research and their inclusion in routine diagnostic protocols for CDH management.
