Gökçeada ve Bozcaada'dan il merkezine 112 ambulanslar ile yapılan nakillerin ekonomik ve sosyal boyutunun değerlendirilmesi

Abstract

Türkiye'de artan acil sağlık hizmetleri talepleri, ambulans gereksiniminde dengesizliklere, acil servislerde yığılmalara, hizmet sunumunda yetersizlikler ile organizasyonda aksaklıklara sebep olabilmektedir. Adalarda özellikle sağlık hizmetlerine ulaşım, zaman, fazladan personel ve para gerektirmektedir. Yerli nüfusun sürekli artması, yaz mevsiminde oluşan nüfus yoğunluğu adalarda sağlık gereksinimini daha fazla artırmaktadır. Bu çalışmasının amacı Gökçeada ve Bozcaada'dan il merkezine 112 ambulanslar ile yapılan hasta nakillerinin kişi, kurum ve devlet bazında ortaya çıkardığı ekonomik ve sosyal boyutu ortaya koymak olmuştur. Araştırmanın evrenini Gökçeada ve Bozcaada'da yaşayanlar oluşturmaktadır. Çalışmanın örneklemi bu adalarda yaşayan ve acil yardım hizmeti kullananlar (18 yaş ve üzeri) olarak belirlenmiştir. Anket sonuçlarına göre önemli sağlık hizmetlerinin il merkezinde olmasının getirdiği ulaşımdaki zorluklar ve ekonomik nedenlerle acil durumdaki hastaların bile tedaviyi reddettiği vakalar tespit edilmiştir. Nakillerde refakatçiye izin verilmemesi de tedavinin reddedilmesinin sebeplerinden birisi olmuştur. Ayrıca acil sağlık hizmetleri bilgisi düzeyinin cinsiyet, yaş, gelir ve eğitim düzeyine göre farklılaşmadığı görülmüştür. Gökçeada ve Bozcaada'dan il merkezine 1 Ocak 2021-30 Eylül 2021 döneminde yapılan 400 sevk - %57,5'i (230) erkek, %42,5'i (170) kadın tespit edilerek incelenmiştir. Tüm sevklerin %33,3'ünü 65 yaş üstü oluşturmuştur. Sevk edilen hastaların %97,5'i Sosyal Güvenlik Kurumu güvencesindedir. Sevk nedenine göre ortopedi ve travmatoloji hastalıkları %28,5 ile birinci sıradadır. Sevklerin en çok gerçekleştiği ay ağustos ayı (%18) ve haziran (13,8) ayıdır. Sevkin en çok hava yolu (hava ambulansı) %37,5 (150) ile gerçekleştiği tespit edilmiştir. Bu güzergâhın toplam ambulans ücreti 2.224.617 TL'dir. Tüm sevklerin toplam ambulans ücreti ise 3.047.530 TL'dir.

The increase in the demand for emergency health services in Turkey may cause imbalances in the need for ambulances, delays in emergency services, inadequacies in service delivery and organizational disruptions. Access to health services, especially on the islands, requires time, extra staff and money. The continuous increase in the local population and the increase in population density in the summer months further increase the health needs of the islands. The aim of this study is to reveal the economic and social dimensions of patient transport from Gökçeada and Bozcaada to the city center by 112 ambulances on the basis of individuals, institutions and states. The population of the research consists of people living in Gökçeada and Bozcaada. The sample of the study was determined as those (18 years and over) living on these islands and using the emergency service. According to the results of the survey, there were cases where even emergency patients refused treatment due to the difficulties in transportation and economic reasons caused by the important health services being in the city centre. Not allowing a companion in transfers has also been one of the reasons for rejection of the treatment. In addition, it was observed that the level of knowledge of emergency health services did not differ according to gender, age, income and education level. 400 dispatches - 57,5% (230) men, 42,5% (170) women made between 1 January 2021 and 30 September 2021 from Gökçeada and Bozcaada to the city center were identified and investigated. Over 65 years old constituted 33,3% of all referrals. 97,5% of the referred patients are under the guarantee of the Social Security Institution. According to the reason for referral, orthopedics and traumatology diseases are in the first place with 28,5%. The months with the highest number of referrals are august (18%) and june (13,8). It was determined that most of the referrals were made by air (air ambulance) 37,5% (150). The total ambulance fee of this route is 2.224.617 TL. The total ambulance fee for all referrals is 3.047.530 TL.
