His electrocardiogram yielded right budle branch block and ST elevation in inferior leads. But the patient had no chest pain. We performed coronary angiography and demonstrated a huge anurysmatic right coronary artery with trombus at the distal part. Medical treatment including aspirine, statin, clopidogrel, heparine and tirofiban was applied to the patient.
